中医药通过SIRT1/AMPK信号通路防治代谢性疾病的研究进展OA
Research progress on traditional Chinese medicine in prevention and treatment of metabolic diseases via SIRT1/AMPK signaling pathway
代谢性疾病是由糖、脂、蛋白质及能量代谢稳态失衡引发的系统性疾病群,涵盖多靶器官损害,已成为全球面临的严峻公共卫生挑战.沉默信息调节因子 1(SIRT1)/腺苷单磷酸活化蛋白激酶(AMPK)信号通路作为细胞能量感应与应激反应的核心调控枢纽,在维持机体内环境稳态中发挥关键作用.该信号轴能够协同促进过氧化物酶体增殖物激活受体 γ 辅激活因子-1α(PGC-1α)等下游大分子的转录,在细胞层面重塑线粒体生物发生、促进脂肪酸氧化、抑制核因子-κB(NF-κB)主导的慢性低度炎症,并诱导受损细胞器自噬.越来越多的证据表明,其功能障碍是贯穿高脂血症、非酒精性脂肪性肝病(NAFLD)、2 型糖尿病(T2DM)、动脉粥样硬化(AS)、多囊卵巢综合征(PCOS)、高尿酸血症(HUA)及痛风、骨质疏松症(OP)等多种代谢性疾病的共性微观机制.研究发现,中医"三焦气化失司与痰瘀互结"的病机演变与 SIRT1/AMPK 通路的失活存在密切的生物学内在联系.多种具有健脾益气、化痰祛瘀功效的中药及其活性单体与经典复方,能够模拟"能量限制"生理效应,靶向激活该通路及相关网络,从而实现多系统代谢紊乱的"异病同治".该文通过梳理 SIRT1/AMPK 通路的分子调控网络,探讨其与代谢性疾病发生发展的病理联系及中医微观演变机制,重点总结中医药干预该靶点的研究策略,并客观分析了当前药效浓度鸿沟及临床转化挑战,为临床"异病同治"理论及其创新药物研发提供理论参考.
Metabolic diseases,characterized by systemic dysregulation of glucose,lipid,protein,and energy homeostasis,encompass a cluster of multisystem disorders and have emerged as a critical global public health challenge.The silent information regulator 1(SIRT1)/adenosine monophosphate-activated protein kinase(AMPK)signaling pathway serves as a central hub for cellular energy sensing and stress response.By synergistically promoting downstream effectors such as peroxisome proliferator-activated receptor γ coactivator-1α(PGC-1α),this signaling axis orchestrates mitochondrial biogenesis,enhances fatty acid β-oxidation,restrains nuclear factor kappa B(NF-κB)-driven chronic low-grade inflammation,and induces indispensable autophagy to maintain intracellular homeostasis.Emerging evidence indicates that the widespread functional impairment of the SIRT1/AMPK pathway acts as a shared microscopic mechanism prevailing across a broad spectrum of metabolic disorders,including obesity,hyperlipidemia,non-alcoholic fatty liver disease(NAFLD),type 2 diabetes mellitus(T2DM),atherosclerosis(AS),polycystic ovary syndrome(PCOS),hyperuricemia(HUA)and gout,and osteoporosis(OP).Remarkably,there thrives a profound biological convergence between the down-regulation of the SIRT1/AMPK circuitry and the pathogenesis cascade delineating the"dysfunction of Sanjiao Qi transformation and intermingling of phlegm and blood stasis"in traditional Chinese medicine(TCM)theories.Various TCM herbs(such as Atractylodes lancea and Poria cocos),active botanical monomers(e.g.,resveratrol,berberine,quercetin),and compound formulations possessing the efficacy of fortifying the spleen,replenishing Qi,resolving phlegm,and removing blood stasis can elegantly mimic"caloric restriction"physiological effects to uniformly and sustainably activate this pathway.This targeted reactivation fundamentally actualizes the classical wisdom of"treating different diseases with the same method"(homotherapy for heteropathy).This review systematically delineates the molecular regulatory network of the SIRT1/AMPK pathway,explores its pathological interplay with disparate metabolic diseases and the corresponding TCM microscopic pathogenesis,and summarizes the latest pharmacological intervention strategies of TCM.Furthermore,it critically evaluates the pre-clinical dilemmas such as"concentration gap"in pharmacokinetics and the imperative need for translational TCM symptom-target mapping,thereby providing robust theoretical paradigms and evidentiary references for clinical precision treatment and innovative drug development.
