术附通痹颗粒缓解类风湿关节炎的药效及基于"脾主肌肉"的抗肌萎缩机制OA
Efficacy of Zhufu Tongbi Granules in alleviating rheumatoid arthritis and its anti-atrophy mechanism based on theory of"spleen governing muscles"
术附通痹颗粒是全国名老中医白长川教授基于"从脾治痹"理论,由经典名方乌头桂枝汤化裁而成的经验方,但其"从脾论治"的现代科学内涵,尤其是对类风湿关节炎(RA)患者常见肌肉萎缩的作用机制尚不明确.该研究整合网络药理学预测与实验验证,首次构建了"方-药-靶-通路-功效"分子网络,识别出肌细胞生成素(Myog)和三重基序蛋白 63(TRIM63)为改善肌肉萎缩的核心调控节点.在完全弗氏佐剂诱导的关节炎(AIA)大鼠模型中,术附通痹颗粒不仅显著缓解关节肿胀,提高痛阈,降低血清白细胞介素(IL)-6、肿瘤坏死因子-α(TNF-α)、IL-17 水平(P<0.05),纠正外周血细胞异常,改善骨微结构破坏,还表现出多靶点的"从脾论治"优势[显著提高胃排空率和肠推进率(P<0.01),纠正贫血状态,增加腓肠肌纤维横截面积和腓肠肌/脑指数(P<0.01)].机制研究发现,该方通过下调 AIA 大鼠腓肠肌中高表达的 Myog 和 TRIM63 蛋白(P<0.05),抑制泛素-蛋白酶体途径介导的肌蛋白降解,且各剂量组均未引起明显肝肾功能损伤.该研究明确了术附通痹颗粒缓解 RA 的综合药效,揭示其基于"脾主肌肉"理论调控 Myog/TRIM63 通路,改善肌萎缩的分子机制,为阐释"从脾治痹"理论的现代内涵及研发兼具抗炎与肌肉保护的 RA 创新中药提供了实验依据.
Zhufu Tongbi Granules is an empirical formula developed from the classic Wutou Guizhi Decoction by the nationally renowned TCM professor BAI Chang-chuan based on the theory of"treating Bi syndrome from the spleen".However,the modern scientific connotation of the"treatment based on regulating the spleen",particularly its mechanism of action on muscle atrophy,which is common in patients with rheumatoid arthritis(RA),remains elusive.This study integrated network pharmacology prediction with experimental validation.For the first time,a molecular network with"formula-herb-target-pathway-efficacy"for this formula was constructed.Myogenin(Myog)and tripartite motif-containing 63(TRIM63)were identified as core regulatory nodes for ameliorating muscle atrophy.In a complete Freund's adjuvant-induced arthritis(AIA)rat model,Zhufu Tongbi Granules not only significantly alleviated joint swelling,increased pain thresholds,reduced the levels of interleukin(IL)-6,tumor necrosis factor-α(TNF-α),and IL-17(P<0.05)in serum,corrected peripheral blood cell abnormalities,as well as ameliorated microstructural destruction of bones,but also demonstrated the multitarget advantages of"treatment based on regulating the spleen".It significantly improved gastric emptying and intestinal propulsion rates(P<0.01),corrected anemic status,and increased both the cross-sectional area of gastrocnemius fibers and the gastrocnemius/brain index(P<0.01).Mechanistic studies reveal that the formula significantly downregulates the overexpression of proteins Myog and TRIM63 in the gastrocnemius of AIA rats(P<0.05),thereby suppressing muscle protein degradation mediated by the ubiquitin-proteasome pathway.Importantly,no significant hepatorenal function injury was observed in the rats of groups with different doses.This study clarified the comprehensive efficacy of Zhufu Tongbi Granules in alleviating RA and revealed its molecular mechanism for improving muscle atrophy via regulating the Myog/TRIM63 pathway based on the theory of"spleen governing muscles".These findings provide experimental evidence for elucidating the modern connotation of the theory of"treating Bi syndrome from the spleen"and developing innovative anti-RA TCM with both anti-inflammatory and muscle-protective properties.
