乌头类药材是中医临床选方常用药,其主要活性与毒性成分双酯型二萜类生物碱(diester diterpenoid alkaloids,DDAs)可诱发快速性心律失常(tachyarrhythmia,TA),严重时将危及生命.近年来,因剂量、配伍或炮制不当导致的中毒事件频发已成为重大公共卫生问题.目前,临床针对DDAs所致TA尚缺乏针对性防治药物,且常规对症治疗不良反应较多,因此寻找安全有效的新型辅助治疗方案迫在眉睫.中医药在防治乌头中毒方面经验丰富,一方面,多种中药单体能通过多靶点机制在疾病网络中发挥协同疗效;另一方面,临方炮制可对DDAs进行结构修饰,实现定向减毒.该文系统综述了国内外中药单体抗乌头类DDAs所致TA的作用机制研究进展,以期为相关解毒药物的研发与临床转化提供理论依据.

Aconitum species are commonly used in traditional Chinese medicine(TCM)clinical practice.Their main active and toxic components,diester diterpenoid alkaloids(DDAs),can induce tachyarrhythmias(TA),which may become life-threatening in severe cases.In recent years,the frequent occur-rence of poisoning incidents caused by improper dosage,formu-lation,or processing has emerged as a major public health con-cern.Currently,targeted therapeutic agents for DDAs induced TA are still lacking in clinical practice,and conventional symp-tomatic treatments often lead to significant adverse effects.Therefore,there is an urgent need to explore safe and effective novel adjuvant therapeutic strategies.TCM offers extensive ex-perience in the prevention and treatment of aconite poisoning.On the one hand,various herbal monomers can exert synergistic therapeutic effects through multi-target mechanisms within dis-ease networks;on the other hand,processing methods can structurally modify DDAs to achieve targeted detoxification.This article systematically reviews recent advances in the mechanisms by which herbal monomers counteract DDAs in-duced TA,aiming to provide a theoretical basis for the develop-ment and clinical translation of novel detoxification strategies.
