基于双眼视功能的角膜塑形术联合视觉训练干预儿童近视的临床效果评价OA
Clinical effect evaluation of corneal reshaping surgery based on binocular visual function combined with visual training in the intervention of children's myopia
目的 分析基于双眼视功能的角膜塑形术(Ortho-K)联合视觉训练(VT)干预儿童近视的临床效果.方法 选取2024年1月至2025年1月我院收治的90例(180眼)近视儿童作为研究对象,随机分为联合组(Ortho-K联合VT干预,n=45)和对照组(单纯Ortho-K干预,n=45).随访12个月,比较两组干预前后的裸眼视力(UCVA)、等效球镜度(SE)、调节滞后(Lag)、眼轴长度(AL)、调节幅度(AMP)、正相对调节(PRA)、负相对调节(NRA)及不良反应发生情况.结果 干预12个月后,联合组的LogMAR和SE绝对值均小于对照组(P<0.05);联合组LogMAR的减小幅度大于对照组(P<0.05),SE绝对值的增加幅度小于对照组(P<0.05).干预12个月后,联合组Lag低于对照组(P<0.05);联合组Lag的减少量大于对照组(P<0.05).干预前及干预12个月后,两组AL差异无统计学意义(P>0.05);联合组AL的增长量小于对照组(P<0.05).干预12个月后,联合组AMP高于对照组(P<0.05);联合组AMP的增长量大于对照组(P<0.05).干预12个月后,联合组PRA绝对值大于对照组(P<0.05),NRA高于对照组(P<0.05);联合组PRA绝对值的增加幅度和NRA的上升幅度均大于对照组(P<0.05).两组均未发生严重不良反应,轻微角膜点染发生率差异无统计学意义(P>0.05).结论 基于双眼视功能的O rtho-K联合VT干预能协同延缓儿童近视进展,有效改善调节功能.
Objective To analyze the clinical effect of corneal reshaping surgery based on binocular visual function(Ortho-K)combined with visual training in the intervention of children's myopia.Methods 90 cases(180 eyes)of chil-dren with myopia admitted to our hospital from January 2024 to January 2025 were selected as the research subjects.They were randomly divided into the combined group(Ortho-K combined with VT intervention,n=45)and the control group(simple Ortho-K intervention,n=45).Follow-up was conducted for 12 months,and the pre-and post-intervention uncorrected visual acuity(UCVA),spherical equivalent(SE),accommodation lag(Lag),axial length(AL),accommodation amplitude(AMP),positive relative accommodation(PRA),negative relative accommodation(NRA),and adverse reactions were compared between the two groups.Results After 12 months of intervention,the absolute values of LogMAR and SE in the combined group were both lower than those in the control group(P<0.05);the reduction in LogMAR in the combined group was greater than that in the control group(P<0.05),and the increase in the absolute value of SE was less than that in the control group(P<0.05).After 12 months of intervention,the Lag in the combined group was lower than that in the control group(P<0.05);the reduction in Lag in the combined group was greater than that in the control group(P<0.05).Before intervention and 12 months after intervention,there was no statistically significant difference in AL between the two groups(P>0.05);the growth in AL in the combined group was less than that in the control group(P<0.05).After 12 months of inter-vention,the AMP in the combined group was higher than that in the control group(P<0.05);the growth in AMP in the combined group was greater than that in the control group(P<0.05).After 12 months of intervention,the absolute value of PRA in the combined group was greater than that in the control group(P<0.05),and NR A was higher than that in the control group(P<0.05);the increase in the abso-lute value of PRA and the increase in NRA in the combined group were both greater than those in the control group(P<0.05).There were no serious adverse reactions in both groups,and there was no statistically significant difference in the incidence of mild corneal staining(P>0.05).Conclusion Ortho-K combined with VT based on binocular visual function intervention can synergistically delay the progression of children's myopia and effectively improve the accommodation func-tion,which is worthy of clinical promotion and application.
