天津地区儿童过敏性结膜炎的过敏原分布特征分析OA
Analysis of the distribution characteristics of allergens in allergic conjunctivitis in children in Tianjin area
目的 探讨天津地区儿童过敏性结膜炎的过敏原分布特征,对临床精准防控提供依据.方法 回顾性分析2024年1月至2025年12月天津市儿童医院眼科确诊的1402例儿童过敏性结膜炎患儿资料,采用Mediwiss过敏原筛查系统检测19种过敏原的总IgE与特异性IgE(sIgE)水平,按年龄、季节分层分析.结果 吸入性过敏原中以霉菌混合物(347,24.75%)、草类(232,16.55%)、猫毛(226,16.12%)居前;食入性过敏原中以牛奶(249,17.76%)、鸡蛋清(80,5.71%)、腰果(66,4.71%)为主.吸入性过敏原总致敏率(712,50.78%),显著高于食入性(367,26.18%)(P<0.05).在不同年龄组间多数吸入及食入过敏率存在显著差异(P<0.05),且随年龄增长,食入性过敏率呈逐渐下降趋势,吸入性过敏率呈逐步上升趋势.季节分布上,食入性过敏原中仅牛奶、芒果的致敏率在不同季节间有显著差异(P<0.05);吸入性过敏原仅苋、户尘螨、桑树、猫毛、草类、树类致敏率存在显著季节差异(P<0.05).结论 天津地区儿童过敏性结膜炎过敏原分布具有明显的年龄、季节特异性,吸入性过敏原占主导地位,霉菌混合物、牛奶为主要致敏原,临床可根据上述特征制定针对性的预防与干预策略.
Objective To explore the distribution characteristics of allergens in children with allergic conjunctivitis in Tianjin and provide a basis for precise clinical prevention and control.Methods A retrospective analysis was conducted on the data of 1402 children diagnosed with allergic conjunctivitis in the Ophthalmology Department of Tianjin Children's Hospital from January 2024 to December 2025.The total IgE and specific IgE(sIgE)levels of 19 allergens were detected using the Mediwiss allergen screening system,and stratified analysis was performed by age and season.Results Among inhalant allergens,mold mixture(347,24.75%),grass(232,16.55%),and cat hair(226,16.12%)were the most common;among food allergens,milk(249,17.76%),egg white(80,5.71%),and cashew(66,4.71%)were the main ones.The total sensitization rate of inhalant allergens(712,50.78%)was significantly higher than that of food allergens(367,26.18%)(P<0.05).There were significant differences in the sensitization rates of inhalant and food allergens among different age groups(P<0.05),and the sensitization rate of food allergens gradually decreased with age,while that of inhalant allergens gradually increased.In terms of seasonal distribution,only the sensitization rates of milk and mango among food allergens showed significant differences among different seasons(P<0.05);among inhalant allergens,only amaranth,house dust mite,mulberry,cat hair,grass,and tree allergens showed significant seasonal differences(P<0.05).Conclusions The distribution of allergens in children with allergic conjunctivitis in Tianjin has obvious age and seasonal specificity,with inhalant allergens being dominant,and mold mixture and milk being the main allergens.Clinically,targeted prevention and intervention strategies can be formulated based on the above charac-teristics.
