血清25(OH)D3、Leptin对早产儿视网膜病变的预测价值OA
The predictive value of serum 25(OH)D3 and Leptin for retinopathy of prematurity in premature infants
目的 探讨血清25羟基维生素D3[25(OH)D3]、瘦素(Leptin)对早产儿视网膜病变(ROP)的预测价值.方法 选取本院ROP患儿80例(ROP组),根据病情程度分为重度组和轻度组,另选取同期健康早产儿80例(对照组).采用Logistic分析、相对危险度分析、ROC曲线分析及Bootstrap内部验证.结果 ROP组较对照组血清25(OH)D3水平低,血清Leptin水平高(P<0.05).重度组较轻度组血清25(OH)D3水平低,血清Leptin水平高(P<0.05).Leptin是ROP的危险因素(P<0.05),25(OH)D3是保护因素(P<0.05).低水平25(OH)D3、高水平Leptin患儿发生ROP的风险为高水平25(OH)D3、低水平Leptin的1.680 倍、1.585 倍.血清 25(OH)D3、Leptin 联合预测发生 ROP 优于单独预测(Z=2.503、2.518,P 均<0.05).预测发生ROP的校准和理想曲线接近一致(H-L检验为x2=7.754,P=0.654).结论 ROP患儿血清25(OH)D3表达降低,Leptin表达升高,且与患儿病情程度有关,联合检测对患儿发生ROP的预测价值较好.
Objective To discuss the predictive value of serum 25 hydroxyvitamin D3[25(OH)D3]and Leptin for retinopathy of prematurity(ROP)in premature infants.Methods 80 cases of ROP in our hospital(ROP group)were selected and divided into severe and mild groups according to the severity of the disease,and 80 cases of healthy preterm infants in the same period(control group)were selected.Logistic analysis,relative risk analysis,ROC curve analysis and Bootstrap internal validation were used.Results The serum 25(OH)D3 level in ROP group was lower than that in control group,and the serum Leptin level was higher(P<0.05).The serum 25(OH)D3 level in severe group was lower than that in mild group,and the serum Leptin level was higher(P<0.05).Leptin was a risk factor for ROP(P<0.05),and 25(OH)D3 was a protective factor(P<0.05).The risk of ROP in children with low level 25(OH)D3 and high level Leptin was 1.680 times and 1.585 times higher than that in children with high level 25(OH)D3 and low level Leptin.The combined prediction of serum 25(OH)D3 and Leptin for the occurrence of ROP is superior to the prediction alone(Z=2.503,2.518,P<0.05).The calibration curve for predicting ROP was close to the ideal curve(H-L test was x2=7.754,P=0.654).Conclusion The expression of serum 25(OH)D3 in children with ROP is decreased,and the expression of Leptin is increased,which is related to the severity of the disease.The combined detec-tion has better predictive value for ROP in children.
