目的:通过实证调研方法,深入探究大数据背景下公立医疗卫生机构病案信息管理的实际状态,为管理模式优化提供数据支撑与实践参考.方法:运用分层整群抽样策略,选取华东、华北、西南等 6 省份 116 家公立医疗机构(三级42 家、二级 59 家、一级及未定级 15 家),综合采用实地调研、问卷调查及深度访谈等手段采集数据,并借助统计分析软件 SPSS 26.0 开展量化研究.结果:管理架构方面,三级医院中 40.48%以独立科室形式设置病案管理部门,二级医院多隶属质控科(占比 35.59%)或信息科(占比23.73%),一级医院及未定级医院 66.67%隶属于医务科.人员配置呈现层级差异,三级医院床位与病案人员配比达 104.58:1,远超国家推荐标准(≤50:1);队伍学历层次随医院级别递减,不同等级医院人员在学历构成上比较差异具有统计学意义(χ2=34.255,P<0.05);三级医院卫生管理与统计学人才占比 14.83%高于二级医院 9.19%、一级及未定级医院2.86%(χ2=5.440、3.844,P<0.05).在病案质量控制与信息安全管理方面,呈现层级差异,质量控制上,终末形式质控普及度最高(93.10%),三级医院实现全覆盖,一级及未定级医院医院占比 80.00%,不同等级医院在开展病案终末形式质控项目开展率上差异有统计学意义(χ2=7.351,P均<0.05);信息安全管理中,以自动记录电子病历修改痕迹开展率最高(66.38%),分级访问控制最低(59.48%).不同等级医院病案归档效率比较,三级医院 3/7 日归档率分别达 92.42%和 97.78%,二级医院为85.30%和 94.27%,一级及未定级医院则降至 79.55%和 90.26%.结论:公立医疗机构病案管理存在架构适配不足、人力负荷失衡、质控体系不完善及信息安全薄弱等问题,需优化管理架构以适应数据治理需求,缓解人力矛盾,强化全流程质控与信息安全建设,推动病案管理向数字化专业化转型.

Objective To deeply explore the actual situation of medical record information management in public medical insti-tutions under the background of big data era through empirical research method,so as to provide data support and practical refer-ence for the optimization of management mode.Methods 116 public medical institutions were chosen from 6 provinces in east Chi-na,north China and southwest China using stratified cluster sampling method.Data were collected through field research,ques-tionnaires and in-depth interview.And quantitative research was conducted using statistical analysis software of SPSS 26.0.Re-sults In terms of management architecture,there were significant differences in affiliation structure of medical record management departments among different level of hospitals.The proportion of medical record management in independent departments was 40.48%in tertiary hospitals,in quality control department and information department was 35.59%and 23.73%respectively in secondary hospitals,and in medical department was 66.67%in primary hospitals.There were hierarchical differences in person-nel allocation.In tertiary hospitals,the ratio of beds to medical record staffs was 104.58:1,far exceeding the national recommen-ded standard(≤50:1).Their educational level was decreased with the hospital level.There were significant differences in edu-cational level composition of staff among different levels of hospitals(χ2=34.255,P<0.05).In terms of quality control and in-formation security management of medical records,there were hierarchical differences.Regarding quality control,popularity rate of terminal quality control was the highest(93.10%).There was full coverage in tertiary hospitals,while the proportion was 80.00%in primary hospitals.There were significant differences in implementation rate of terminal quality control among different levels of hospitals(χ2=7.351,P<0.05).In information security management,implementation rate of automatically recording modification traces in electronic medical records was the highest(66.38%),while the implementation rate of hierarchical access control was the lowest(59.48%).In terms of medical records archiving efficiency,3d and 7d archiving rates in tertiary hospi-tals,secondary hospitals,primary or ungraded hospitals was(92.42%,97.78%),(85.30%,94.27%)and(79.55%,90.26%),respectively.Conclusion There are problems such as insufficient architecture adaptation,unbalanced human resource load,imperfect quality control system and weak information security in medical record management of public medical institutions.It is necessary to optimize the management architecture to adapt to data governance demand,alleviate human resource contradic-tion,strengthen whole-process quality control and information security construction,and promote digitalization and professional-ization transformation of medical record management.
