目的:探究整合素β3(Integrin β3,ITGB3)在增生性瘢痕(Hypertrophic Scar,HS)中的功能及作用机制.方法:收集2022年5月-2023年5月西安市中心医院整形外科收治并确诊为HS患者的增生性瘢痕组织标本共30例,同期收集手术切除的正常皮肤组织标本30例.采用HE染色和Masson染色检测增生性瘢痕组织病变和纤维化程度,免疫组织化学法检测增生性瘢痕组织中ITGB3的表达.将人增生性瘢痕成纤维细胞(Huma Hypertrophic Scar Fibroblast,hHSF)分为对照组、sh-NC组(hHSF细胞感染sh-NC),sh-ITGB3组(hHSF细胞感染sh-ITGB3),SRI-011381组(hHSF细胞感染sh-ITGB3,并采用TGF-β1激活剂SRI-011381处理).采用CCK-8法和EdU荧光标记法检测细胞增殖水平,流式细胞术检测细胞凋亡水平,细胞划痕实验检测细胞迁移水平.采用qRT-PCR分析组织和细胞中ITGB3基因表达水平,Western blot分析组织和细胞中ITGB3、Collagen Ⅰ、α-SMA、MMP-2、TIMP-2、TGF-β1、p-Smad2和Smad2蛋白表达水平.结果:正常皮肤组织表皮和真皮分界明显,胶原纤维排列整齐、密度均匀;瘢痕组织中真皮层明显增厚,存在大量排列紊乱无序的胶原纤维.与正常皮肤组织比较,瘢痕组织中ITGB3的mRNA和蛋白表达水平升高(P<0.05),TGF-β1、Collagen Ⅰ、α-SMA和TIMP-2蛋白表达水平以及p-Smad2/Smad2比值均升高(P<0.05),MMP-2蛋白表达水平降低(P<0.05).与hSF细胞比较,hHSF细胞中ITGB3的mRNA和蛋白表达水平均升高(P<0.05).与对照组或sh-NC组比较,sh-ITGB3组和SRI-011381组细胞中ITGB3的mRNA和蛋白表达水平均降低(P<0.05).与对照组或sh-NC组比较,sh-ITGB3组细胞增殖水平和相对迁移率均降低(P<0.05),细胞凋亡水平均升高(P<0.05),TGF-β1、Collagen Ⅰ、α-SMA、TIMP-2蛋白表达水平和p-Smad2/Smad2比值均降低(P<0.05),MMP-2蛋白表达水平升高(P<0.05).与sh-ITGB3组比较,SRI-011381组细胞增殖水平和相对迁移率均升高(P<0.05),细胞凋亡水平降低(P<0.05),TGF-β1、Collagen Ⅰ、α-SMA、TIMP-2蛋白表达水平和p-Smad2/Smad2比值均升高(P<0.05),MMP-2蛋白表达水平降低(P<0.05).结论:ITGB3在瘢痕组织中高表达,可能是通过激活TGF-β1/Smad2信号通路促进hHSF细胞增殖和迁移以及ECM沉积,抑制hHSF细胞凋亡,进而促进增生性瘢痕的形成.

Objective To explore the role and mechanism of Integrin β3(ITGB3)in Hypertrophic scar(HS).Methods A total of 30 hyperplastic scar tissue specimens from patients diagnosed with HS in the Department of orthopedic in Xi'an Central Hospital from May 2022 to May 2023 were collected,and 30 normal skin tissue specimens from surgically removed patients were collected during the same period.The degree of lesion and fibrosis in hypertrophic scar tissue were detected by HE staining and Masson staining.ITGB3 expression in hypertrophic scar tissue was detected by immunocytochemistry.Human hypertrophic scar fibroblasts(hHSFs)was divided into control group,sh-NC group(hHSF cells were infected with sh-NC),and sh-ITGB3 group(hHSF cells were infected with sh-ITGB3),and SRI-011381 group(hHSF cells were infected with sh-ITGB3 and treated with TGF-β1 activator SRI-011381).The cell proliferation was detected by CCK-8 method and EdU fluorescence labeling method.The cell apoptosis was detected by flow cytometry,and cell migration was detected by cell scratch assay.ITGB3 gene expression levels in tissues and cells were analyzed by qRT-PCR.while protein expression levels of ITGB3,Collagen I,α-SMA,MMP-2,TIMP-2,TGF-β1,p-Smad2 and Smad2 in tissues and cells were analyzed by Western blot.Results The boundary between epidermis and dermis was obvious in normal skin tissue,and the collagen fibers were arranged neatly with uniform density.The dermis in HS tissue was obviously thickened and there were a large number of disordered and irregularly arranged collagen fibers.Compared with normal skin tissues,mRNA and protein expression levels of ITGB3 in HS tissues were increased(P<0.05),protein expression levels of TGF-β1,Collagen I,α-SMA and TIMP-2 and p-Smad2/Smad2 ratio were increased(P<0.05).And the expression level of MMP-2 protein was decreased(P<0.05).Compared with hSF cells,the mRNA and protein expression levels of ITGB3 in hHSF cells were increased(P<0.05).Compared with the control group or sh-NC group,the mRNA and protein expression levels of ITGB3 in sh-ITGB3 group and SRI-011381 group were decreased(P<0.05).Compared with control group or sh-NC group,the cell proliferation level and relative cell migration rates in the sh-ITGB3 group were decreased(P<0.05),while the cell apoptosis level was increased(P<0.05),the TGF-β1,Collagen I,α-SMA,TIMP-2 protein expression level and p-Smad2/Smad2 ratio were decreased(P<0.05),MMP-2 protein expression level was increased(P<0.05).Compared with sh-ITGB3 group,the cell proliferation level and relative cell migration rates in the SRI-011381 group were increased(P<0.05),while the cell apoptosis level was decreased(P<0.05),the TGF-β1,Collagen I,α-SMA,TIMP-2 protein expression level and p-Smad2/Smad2 ratio were increased(P<0.05),MMP-2 protein expression level was decreased(P<0.05).Conclusion ITGB3 is highly expressed in HS tissues,which may promote the hHSF cells proliferation and migration and ECM deposition through activation of the TGF-β1/Smad2 signaling pathway,and inhibit the apoptosis of hHSF cells,thus promoting the formation of hypertrophic scars.
