734例次感染性疾病会诊病例的临床药师干预效果与影响因素OA
Effectiveness of clinical pharmacist interventions and influencing factors in 734 consultation cases of infectious diseases
目的 评价临床药师参与感染性疾病会诊的干预效果,并分析影响抗感染治疗结局的相关因素,为优化药学服务提供依据.方法 对某院2024年1-12月临床药师参与抗感染会诊的病例进行回顾性分析,以临床药师抗感染会诊意见采纳率作为主要指标,临床治疗有效率作为结局指标.根据会诊意见采纳情况分为采纳组和未采纳组,比较2组患者的临床治疗结局.采用多因素logistic回归模型分析影响临床治疗结局(治疗有效和无效)的相关因素.结果 最终纳入734例次感染性疾病会诊病例,临床药师抗感染会诊目的以抗感染药物选择为主,占74.39%.有635例次抗感染药学会诊意见被采纳,会诊意见采纳率为86.51%,采纳组与未采纳组的治疗有效率差异具有统计学意义(86.68%vs 26.67%,P<0.05).多因素logistic回归分析结果显示,低蛋白血症和肿瘤是临床药师抗感染会诊治疗无效的危险因素,会诊类型为单学科会诊、获得病原学检测结果和采纳临床药师的会诊意见是保护因素.结论 临床药师参与感染性疾病会诊对抗感染治疗方案制订及患者临床结局改善具有重要作用.
AIM To evaluate the effectiveness of clinical pharmacist involvement in infectious disease consultations and to analyze factors influencing antimicrobial treatment outcomes,providing evidence for optimizing pharmaceutical services.METHODS A retrospective analysis was conducted on cases involving clinical pharmacists in anti-infective consultations at a hospital between January and December 2024.The acceptance rate of suggestions raised by clinical pharmacists in anti-infective consultation was used as the primary measure,and the clinical response rate was used as the study outcome.Patients were divided into an adoption group and a non-adoption group based on whether the consultation recommendations were adopted,and the clinical outcomes were compared between 2 groups.The multivariate logistic regression model was employed to analyze factors influencing clinical treatment outcomes(treatment efficacy and treatment inefficacy).RESULTS A total of 734 cases of infectious disease consultations were ultimately included.The purpose of clinical pharmacist consultation for anti-infective therapy primarily revolved around the selection of anti-infective drugs,accounting for 74.39%.A total of 635 anti-infective treatment consultation recommendations were adopted,with an adoption rate of 86.15%.The treatment efficacy rate in the adoption group demonstrated a statistically significant difference compared to the non-adoption group(86.68%vs 26.67%,P<0.05).Multivariate logistic regression analysis identified that hypoalbuminemia and tumor diseases emerged as statistically validated risk factors for treatment inefficacy in anti-infective consultations involving clinical pharmacists,whereas single-discipline consultations,availability of microbiological testing,and adoption of clinical pharmacists' recommendations demonstrated significant protective effects correlated with therapeutic success(P<0.05).CONCLUSION Clinical pharmacists' participation in infectious disease consultations plays a significant role in optimizing therapeutic protocols and improving patient clinical outcomes.
