目的 比较碘克沙醇注射液仿制药与原研药应用于冠状动脉CT血管成像(CCTA)检查中的图像质量,为碘克沙醇注射液仿制药的临床等效性验证提供影像学证据.方法 纳入2022年1月1日至10月31日使用碘克沙醇注射液行CCTA的患者,按照碘克沙醇注射液来源分为仿制组(n=86)与原研组(n=92).通过Likert评分法对冠状动脉左主干(LM)、左前降支(LAD)、左回旋支(LCX)及右冠状动脉(RCA)进行影像质量主观评价;通过测量各血管管腔及邻近脂肪CT值和噪声标准差(SD)值,计算信噪比(SNR)和对比噪声比(CNR)进行影像质量客观评价.结果 仿制组与原研组Likert评分≥3分的冠状动脉节段占比均>95%,组间差异无统计学意义(P>0.05).2组冠状动脉的CT值、SD值、SNR和CNR均达临床诊断阈值.仿制组在LM、LAD和RCA冠状动脉上的CT值均显著高于原研组(P<0.05),且在LM、LCX上的SD值也显著高于原研组(P<0.05),但SNR与CNR在所有血管测量区域内差异均无统计学意义(差值的95%CI均包含0,P>0.05,r值0~0.13).结论 碘克沙醇仿制药在影像质量上与原研药等效,能满足冠状动脉病变的诊断需求,支持其作为替代品种选择.

AIM To compare the image quality between generic and original iodixanol injections in coronary computed-tomography angiography(CCTA),providing imaging evidence for validating the clinical equivalence of the generic product.METHODS Patients who underwent CCTA using iodixanol injection between January 1,2022,and October 31,2022,were enrolled and divided into a generic group(n=86)and an original group(n=92).Subjective image quality assessment was performed using the Likert scale for the left main coronary artery(LM),proximal left anterior descending artery(LAD),proximal left circumflex artery(LCX),and proximal right coronary artery(RCA).Objective metrics,including CT values,standard deviation(SD),signal-to-noise ratio(SNR),and contrast-to-noise ratio(CNR),were measured in the luminal and adjacent adipose regions of LM,LAD,LCX,and RCA.RESULTS Both groups exhibited subjective image quality scores ≥ 3 for all coronary segments(over 95%),with no significant intergroup differences(P>0.05).The CT value,SD value,SNR and CNR of the 2 groups all reached the clinical diagnostic threshold.The LM-CT values,LAD-CT values,RCA-CT values,LM-SD values,and LCX-SD values in the generic group were significantly higher than those in the original group(all P<0.05).However,there were no statistically significant differences in SNR and CNR,across all vascular territories(the 95%CIs of all differences included 0,with P>0.05,and effect sizes r ranged from 0 to 0.13).CONCLUSION The generic iodixanol injection demonstrates equivalent image quality to the original product in coronary artery imaging,supporting its role as a cost-effective alternative for diagnosing coronary pathologies.
