目的 探讨PDCA循环联合根因分析(RCA)法在提高门诊药房药品追溯码扫码率、准确率及发药速度中的应用效果.方法 于2025年5-7月在本院门诊药房开展三阶段研究:基线调查(5月)、干预实施(6月)、效果评价(7月).研究采用瑞士奶酪模型进行RCA,针对追溯码扫码环节的漏洞实施PDCA循环管理,包括优化设备配置、再造工作流程和强化培训考核等措施.结果 干预后药品追溯码扫码率从21.2%提升至99.6%(P<0.05),扫码准确率从73.5%提高至99.8%(P<0.05),患者取药等待时间由742.75 s减少为679.33 s.结论 PDCA循环联合RCA法能有效提升药品追溯码扫码率和准确率,且加快发药速度,为门诊药房精细化管理提供科学路径.

AIM To investigate the application effect of the PDCA cycle combined with root cause analysis(RCA)in improving the scanning rate,accuracy,and dispensing speed of drug traceability codes in outpatient pharmacies.METHODS A three-stage study was conducted in the outpatient pharmacy of our hospital from May to July 2025:baseline survey(May),intervention implementation(June),and effect evaluation(July).The Swiss cheese model was used for root cause analysis,and PDCA cycle management was implemented to address vulnerabilities in the traceability code scanning process,including optimizing equipment configuration,reengineering the workflow,and strengthening training and assessment.RESULTS Following the implementation of advanced drug traceability systems,the scanning rate of drug traceability codes surged from 21.2%to 99.6%(P<0.05).Additionally,the accuracy of these scans improved from 73.5%to 99.8%(P<0.05).The waiting time for drug collection also saw a reduction,from 742.75 seconds to 679.33 seconds.CONCLUSION PDCA cycle,combined with root cause analysis,can effectively improve the scanning rate and accuracy of drug traceability codes,speed up drug dispensing,and provide a scientific path for the refined management of outpatient pharmacies.
