基于价值流图技术的静脉用药调配中心工作流程优化研究OA
A study on workflow optimization of pharmacy intravenous admixture service based on value stream mapping
目的 探讨将精益管理工具——价值流图技术应用于静脉用药调配中心(PIVAS)流程优化的实施路径,并评估其在提升运营效率与质量控制方面的实际效果.方法 依据价值流图技术框架,依次开展"绘制当前价值流图""识别浪费""设计未来价值流图""实施改进方案""持续改进"5个步骤,对PIVAS现行工作流程进行系统性优化.通过量化分析主要绩效指标(如人力成本、生产周期、价值流效率、生产效率和调配精准度等),评估PIVAS工作流程改进后的运营效果.结果 当前价值流图识别出主要瓶颈工序为"加药混合"环节,同时存在加工过早、动作冗余、过度加工和工序间衔接不畅等多类非增值活动.在此基础上,设计未来价值流图并实施多项精益改进措施,包括引入半自动化设备、流程重构、标准作业制定与岗位整合等.对PIVAS工作流程进行优化后,运营效果得到了显著提升:人力成本降低20.00%,在制品数量下降13.84%,生产周期缩短28.57%,价值流效率提升18.49%,生产效率提升75.06%,调配精准度提高0.05%.结论 作为一种可视化、系统化的精益管理工具,价值流图技术能够有效识别流程瓶颈、挖掘非增值环节,助力实现成本控制、效率提升与质量保障的多重目标,具有广泛的推广和应用潜力.
AIM To explore the application of the lean management tool,value stream mapping(VSM),in optimizing workflows of the pharmacy intravenous admixture service(PIVAS),and to evaluate its effectiveness in improving operational efficiency and quality control.METHODS Based on the VSM analytical framework,a five-step approach was employed:current state mapping,waste identification,future state design,implementation of improvements,and continuous refinement.A systematic optimization of the PIVAS workflow was conducted.The impact of the changes was assessed using key performance indicators,including labor cost,production cycle time,value stream efficiency,and admixture accuracy.RESULTS The current state map revealed the primary bottleneck at the drug mixing stage,along with multiple non-value-added activities such as premature processing,unnecessary motions,excessive processing,production efficiency,and inefficient handoffs between process stages.Based on these findings,a future state map was developed and a series of lean interventions were implemented,including the introduction of semi-automated equipment,process reengineering,standardization of operations,and role consolidation.Post-optimization metrics showed notable improvements:labor costs decreased by 20.00%,work-in-process inventory decreased by 13.84%,the production cycle time shortened by 28.57%,value stream efficiency increased by 18.49%,production efficiency improved by 75.06%,and admixture accuracy improved by 0.05%.CONCLUSION As a visual and systematic lean management tool,VSM can effectively identify process bottlenecks and uncover non-value-adding activities,helping to achieve multiple goals including cost control,efficiency improvement,and quality assurance,and holds broad potential for promotion and application.
