桡骨远端骨折累及桡骨背侧患者术后骨块移位影响因素分析OA
Analysis of risk factors of postoperative bone fragment displacement in patients with distal radius fractures involving the dorsal side of the radius
目的:探讨桡骨远端关节内骨折累及桡骨背侧患者掌侧内固定术后背侧关节内骨块移位的影响因素,为临床治疗提供理论依据.方法:回顾性分析2022年2月至2024年6月接受掌侧内固定术的64例桡骨远端关节内骨折患者的临床资料,其中男22例,女42例;年龄35~79(61.71±10.25)岁.收集患者年龄、性别、骨折AO分型、术前CT影像学参数、术后康复情况及随访结果等数据,术后背侧关节内骨块移位的影响因素采用单/多因素Logistic回归分析,以受试者工作特征(receiver operating characteristic,ROC)曲线评价模型效能.结果:64例桡骨远端骨折患者术后,18例发生骨块移位,发生率为28.13%(18/64).单因素Logistic回归分析结果显示,移位组和未移位组比较,骨质疏松、术前骨块总移位距离、横向宽度、横纵比及体积,差异有统计学意义(P<0.05).多因素分析结果显示,术前骨块总移位距离[OR=1.864,95%CI(1.030,3.372),P=0.040]、骨质疏松[OR=7.767,95%CI(1.333,45.108),P=0.022]、术前骨块横纵比[OR=0.393,95%CI(0.227,0.681),P=0.001]、术前骨块横向宽度[OR=0.624,95%CI(0.391,0.996),P=0.048]和术前骨块体积[OR=0.980,95%CI(0.963,0.998),P=0.026]是影响术后背侧关节内骨块移位的独立因素.ROC曲线分析结果显示,术前骨块总移位距离、骨质疏松、横纵比、横向宽度和体积的曲线下面积(area under the curve,AUC)均低于联合预测的AUC.结论:桡骨远端关节内骨折累及桡骨背侧患者术后骨块移位受多种因素影响,其中患者存在骨质疏松、术前骨块总移位距离、横纵比、横向宽度和体积是重要的独立影响因素.临床医生应全面评估患者骨折块的稳定性,选择合适的内固定方法,以降低术后骨折块移位的风险.
Objective To analyze the factors influencing dorsal intra-articular bone fragment displacement after palmar internal fixation in patients with distal radius intra-articular fractures involving the dorsal radius,providing a theoretical basis for clinical treatment.Methods A retrospective analysis was conducted on the clinical data of 64 patients with intra-articular distal radius fractures who underwent volar internal fixation between February 2022 and June 2024,including 22 males and 42 females,with an age range of 35 to 79 years and a mean age of(61.71±10.25)years.Data including patients' age,gender,AO fracture classification,preoperative CT imaging parameters,postoperative rehabilitation status and follow-up outcomes were collected.Univariate and multivariate Logistic regression analyses were used to identify the influencing factors of postoperative dorsal intra-articular bone fragment displacement,and receiver operating characteristic(ROC)curve was adopted to evaluate the efficacy of the prediction model.Results Among 64 patients who underwent surgery for distal radius fractures,fragment displacement occurred in 18 cases,with an incidence rate of 28.13%(18/64).Univariate Logistic regression analysis demon-strated that there were statistically significant differences between the displacement group and the non-displacement group re-garding osteoporosis,preoperative total fragment displacement distance,transverse width,transverse-to-longitudinal ratio,and volume(P<0.05).The results of multivariate analyses showed that preoperative total distance of fracture block displacement[OR=1.864,95%CI(1.030,3.372),P=0.040],osteoporosis[OR=7.767,95%CI(1.333,45.108),P=0.022],transverse-to-lon-gitudinal ratio[OR=0.393,95%CI(0.227,0.681),P=0.001],transverse width[OR=0.624,95%CI(0.391,0.996),P=0.048],and volume[OR=0.980,95%CI(0.963,0.998),P=0.026]were the independent influences on postoperative dorsal intra-artic-ular fracture block displacement(P<0.05).The results of ROC curve analyses showed that the area under the curve(AUC)of osteoporosis,preoperative fracture block displacement distance,transverse width,transverse to longitudinal ratio,volume were lower than the AUC of joint prediction,respectively.Conclusion Postoperative fracture block displacement in patients with in-tra-articular fractures of the distal radius involving the dorsal radius is influenced by a number of factors,among which the presence of osteoporosis in the patient,total preoperative fracture block displacement distance,transverse-to-longitudinal ratio,transverse width,and volume are important independent influencing factors.Clinicians should take these factors into account to thoroughly assess the stability of the patient's fracture block and select the most appropriate internal fixation method to reduce the risk of postoperative block displacement.
