Hurley Ⅱ期肛周化脓性汗腺炎的诊断困境与健康行为干预(附1例报告及文献复习)OA

Objective To investigate the clinical characteristics,diagnostic points,therapeutic strategies and the importance of health behavior intervention in perianal hidradenitis suppurativa(HS).Methods The diagnosis,treatment and prognosis of a 41-year-old male patient with Hurley stage Ⅱ perianal HS were reported.Relevant Chinese and English literatures from 2010 to 2025 were retrieved and reviewed with the key words of hidradenitis suppurativa,differential diagnosis,health behavior intervention,surgery and case report.Results The patient was admitted with recurrent perianal swelling,pain and pus discharge for 2 years.He was diagnosed with Hurley stage Ⅱ perianal HS by clinical examination and perianal MRI.Extended lesion resection and debridement were performed,and postoperative pathology confirmed chronic inflammation with sinus tract formation.At 6-month follow-up,the wound healed well without recurrence.Literature review showed that MRI is one of the important examinations for differentiating HS from perianal Crohn's dis-ease,surgical treatment is effective for moderate to severe HS,and lifestyle intervention and long-term manage-ment should be emphasized.Conclusion Perianal HS is prone to misdiagnosis.Detailed medical history,specialized physical examination combined with imaging examination are the basis for correct diagnosis.For Hurley stage Ⅱ patients,active surgical intervention can achieve a favorable prognosis.Meanwhile,multidisci-plinary cooperation and individualized long-term management are crucial for diagnosis and prognosis.
