自然杀伤细胞在卵巢癌免疫治疗中的研究现状及展望OA

Research progress and future perspectives of natural killer cells in ovarian cancer immunotherapy

卵巢癌是死亡率最高的妇科恶性肿瘤.尽管肿瘤细胞减灭术联合铂类-紫杉醇类药物的一线化疗已使部分患者的生存预后有所改善,但是多数患者仍会出现铂类药物耐药复发,因此亟待探索新的治疗策略以实现疾病的长期缓解.近年来,基于自然杀伤(natural killer,NK)细胞的免疫治疗凭借其高效的杀伤活性和可靠的安全性,在卵巢癌治疗中展现出良好应用前景.本综述系统讨论卵巢癌肿瘤微环境(tumor microenvironment,TME)通过抑制性细胞因子富集、代谢重编程及诱导NK细胞表面抑制性免疫检查点过表达等方式损害NK细胞功能的机制,重点阐述细胞因子治疗、嵌合抗原受体 NK(chimeric antigen receptor-NK,CAR-NK)细胞及NK细胞衔接器(NK cell engagers,NKCEs)等治疗方案及其临床研究的进展,同时总结目前基于NK细胞的卵巢癌免疫治疗临床试验概况,并对未来联合治疗策略的研究方向进行展望,以期为基于NK细胞的免疫治疗策略优化提供新思路.

Ovarian cancer remains the most lethal gynecologic malignancy.Although cytoreductive surgery and platinum-paclitaxel combination chemotherapy as first-line treatment have improved outcomes for a subset of patients,the majority eventually develop platinum-resistant recurrence,underscoring the urgent need for novel therapeutic strategies to achieve durable disease control.Recent advances in natural killer(NK)cell-based immunotherapy have demonstrated substantial potential for ovarian cancer treatment,capitalizing on the potent cytotoxicity and favorable safety profile of NK cells.This review systematically discusses how the ovarian cancer tumor microenvironment(TME)impairs NK-cell function through enrichment of inhibitory cytokines,metabolic reprogramming,and overexpression of inhibitory immune checkpoints on the NK-cell surface.It then focuses on current therapeutic strategies and their clinical development,including cytokine-based therapies,chimeric antigen receptor-NK(CAR-NK)cells and NK cell engagers(NKCEs).We further summarize the landscape of completed and ongoing clinical trials involving NK-cell-based immunotherapy for ovarian cancer,and discuss future directions for rational combination strategies,so as to provide updated insights to optimize NK cell-based immunotherapeutic approaches.
