基于决策曲线分析血清CCL2、PDK4、YY1对急诊重症肺炎患儿并发心肌损害的预测价值OA
The predictive value of serum CCL2,PDK4,and YY1 for myocardial damage in children with severe emergency pneumonia based on decision curve analysis
目的 探讨血清 CC 趋化因子配体2(CCL2)、丙酮酸脱氢酶激酶4(PDK4)、转录因子阴阳1(YY1)水平对急诊重症肺炎患儿并发心肌损害的预测价值.方法 选取 2025 年 1-12 月昆明市儿童医院急诊内科收治的重症肺炎患儿 215 例作为研究对象,根据入院后 7d 内是否并发心肌损害,将患儿分为心肌损害组 62 例和无心肌损害组 153 例.采用酶联免疫吸附法检测血清 CCL2 水平,采用实时荧光定量 PCR 检测血清 PDK4、YY1 水平;多因素Logistic回归分析重症肺炎患儿并发心肌损害的影响因素;受试者工作特征(ROC)曲线分析血清 CCL2、PDK4、YY1 水平对重症肺炎患儿并发心肌损害的预测价值,并绘制决策曲线(DCA)评估不同阈值概率下预测模型的临床净收益;相对危险度分析血清 CCL2、PDK4、YY1 水平与重症肺炎患儿并发心肌损害的关联强度.结果 与无心肌损害组比较,心肌损害组血清 CCL2、PDK4 水平升高,血清 YY1 水平降低(t/P=9.123/<0.001、8.700/<0.001、8.555/<0.001).多因素 Logistic 回归分析显示,呼吸衰竭、C 反应蛋白(CRP)高、肌酸激酶同工酶 MB(CK-MB)高、CCL2 高、PDK4 高是重症肺炎患儿并发心肌损害的独立危险因素[OR(95%CI)=3.574(1.064~12.001)、2.104(1.115~3.970)、2.060(1.171~3.623)、2.811(1.498~5.274)、4.319(1.543~12.086)],YY1 高是独立保护因素[OR(95%CI)=0.338(0.131~0.869)].血清 CCL2、PDK4、YY1 水平单独及联合预测重症肺炎患儿并发心肌损害的曲线下面积(AUC)分别为 0.806、0.783、0.826、0.895,三者联合预测价值高于单项指标(Z/P=2.677/0.007、3.375/0.001、2.116/0.034);当阈值概率在0.10~0.85的临床决策区间内,CCL2、PDK4、YY1 三者联合预测模型的临床净获益显著高于单一指标模型.血清CCL2、PDK4 高水平组患儿并发心肌损害的风险分别是低水平组的 1.629、1.733 倍,血清 YY1 高水平组患儿并发心肌损害的风险是低水平组的 0.616 倍(P<0.05).结论 血清 CCL2、PDK4 水平升高及 YY1 水平降低是急诊重症肺炎患儿并发心肌损害的独立危险因素,三者联合检测对心肌损害具有良好的预测效能,具备较高的临床应用潜力.
Objective To explore the predictive value of serum CC chemokine ligand 2(CCL2),pyruvate dehydro-genase kinase 4(PDK4),and Yin Yang 1 transcription factor(YY1)for myocardial injury in children with severe pneumonia in the emergency department.Methods A total of 215 children with severe pneumonia admitted to the Emergency Internal Medicine Department of Kunming Children's Hospital from January 2025 to December 2025 were selected as study subjects.According to whether they developed myocardial injury within 7 days after admission,they were divided into a myocardial in-jury group(n=62)and a non-myocardial injury group(n=153).Clinical data of the children were collected,and serum levels of CCL2,PDK4,and YY1 were measured within 24 hours of admission.Independent risk factors for the development of myo-cardial injury in children with severe pneumonia were analyzed.The predictive efficacy of serum CCL2,PDK4,and YY1 alone and in combination for myocardial injury was evaluated,and the clinical utility of the prediction model was assessed using decision curve analysis.Risk analysis was performed to examine the strength of the association between serum levels of CCL2,PDK4,YY1 and the development of myocardial injury in pediatric emergency patients with severe pneumonia.Results The incidence of myocardial injury among the 215 children with severe pneumonia was 28.84%(62/215).Serum levels of CCL2 and PDK4 in the myocardial injury group were significantly higher than those in the non-myocardial injury group,while YY1 level was significantly lower(t=9.123,8.700,8.555,respectively,all P<0.001).Respiratory failure,elevated C-re-active protein(CRP),elevated creatine kinase-MB(CK-MB),elevated CCL2,and elevated PDK4 were independent risk factors for myocardial injury in children with severe pneumonia,while elevated YY1 was an independent protective factor[OR(95%CI)=3.574(1.064-12.001),2.104(1.115-3.970),2.060(1.171-3.623),2.811(1.498-5.274),4.319(1.543-12.086),0.338(0.131-0.869)].The AUCs of CCL2,PDK4,YY1,and their combination for predicting myocardial injury were 0.806,0.783,0.826,and 0.895,respectively.The combined model had a significantly larger AUC(DeLong method:Z/P=2.677/0.007,3.375/0.001,2.116/0.034).Within the threshold probability range of 0.10-0.85,the combined prediction model showed higher clinical net benefit(P<0.05).Conclusion Elevated serum CCL2 and PDK4 levels,as well as decreased YY1 levels,are inde-pendent risk factors for the development of myocardial injury in pediatric emergency patients with severe pneumonia.Com-bined detection of these three markers demonstrates good predictive value and favorable potential for clinical application.
