胃镜直视下柱状气囊扩张联合金属支架置入治疗上消化道恶性梗阻及难治性良性梗阻的临床研究OA

Objective:To investigate the efficacy and safety of cylindrical balloon dilatation combined with self-ex-panding metal stent implantation under gastroscope in the treatment of malignant obstruction and refractory benign obstruction of upper digestive tract.Methods:From July 2020 to May 2024,35 patients with upper digestive tract obstruction in Jiuquan Hospital of Shanghai First People's Hospital were retrospectively analyzed.According to the way of expansion,they were divided into observation group(18 cases)and control group(17 cases).The surgical indexes,clinical outcome,swallowing function and complications were compared between the two groups.Results:Compared with the control group,the observation group had fewer dilation times,longer operation time,higher success rate of one-time stent positioning and lower long-term restenosis rate(P<0.05).At 3 and 6 months af-ter operation,the dysphagia score in the observation group was lower(P<0.05).There was no significant differ-ence in the total incidence of complications between the two groups(P>0.05).Conclusion:The treatment of upper gastrointestinal obstruction with cylindrical balloon expansion combined with self-expanding metal stent implanta-tion under gastroscope has higher stent positioning accuracy and lower long-term restenosis rate,and also has ob-vious effect on improving swallowing function of patients.
