[背景]呼和浩特属北方温带半干旱气候,植被多样,气传花粉是过敏性鼻炎重要诱因,且浓度季节性变化显著.现有多地研究已证实花粉短期暴露与鼻炎就诊相关,本研究立足本地时序数据,探究花粉暴露滞后健康效应. [目的]定量评估呼和浩特市气传花粉与居民每日过敏性鼻炎就诊门诊量的暴露-反应关系,识别并量化单日与累积滞后效应及关键滞后时间. [方法]收集 2023年 1月 1日—2024年 9月 30日呼和浩特市每日过敏性鼻炎门诊量、花粉浓度、大气污染物浓度及气象数据.采用广义加性模型,定量分析花粉暴露水平与过敏性鼻炎门诊量之间的暴露-反应关系. [结果]2023-2024年呼和浩特市花粉浓度均呈双峰分布.单日滞后效应分析显示:不同花粉时期对过敏性鼻炎门诊量的滞后效应值均在 lag2时达到最大.花粉浓度每增加一个四分位数,秋季期过敏性鼻炎门诊量的超额就诊风险最大,ER为 10.52%(95%CI:2.97%~18.62%);累积滞后效应分析显示:采样全时期和春季期的效应值均在 lag01时达到最大,秋季期的超额就诊风险在 lag02时最大,低浓度期的超额就诊风险在 lag03时最大,ER分别为 3.43%(95%CI:2.44%~4.42%)、2.80%(95%CI:0.60%~5.06%)、7.55%(95%CI:3.34%~11.93%)和4.22%(95%CI:1.96%~6.52%),且累积滞后效应随累积滞后时间的增加而降低.花粉与过敏性鼻炎门诊量的暴露-反应关系在全时期呈低浓度陡峭、高浓度平缓的非线性关系,在春、秋季期均呈有阈值的线性关系. [结论]呼和浩特市花粉暴露水平升高与过敏性鼻炎门诊量显著相关,影响以短期急性效应为主且存在时期特异性;单日滞后效应峰值均在 lag2,采样全时期、春季期、秋季期累积滞后效应峰值分别位于 lag01、lag01、lag02,滞后效应随滞后天数快速回落,可为当地花粉健康预警与易感人群防护提供时序依据.

[Background]Airborne pollen is a major trigger of allergic rhinitis,and its concentration exhibits significant seasonal variations in the north temperate semi-arid climate of Hohhot.While studies in many regions have confirmed an association between short-term pollen exposure and rhinitis-related outpatient visits,local epidemiological evidence remains limited.This study examines the lagged health effects of pollen exposure using local time-series data. [Objective]To quantitatively assess the exposure-response relationship between airborne pollen and daily outpatient visits for allergic rhinitis in Hohhot,and to identify and quantify the single-day and cumulative lag effects,as well as key lag periods. [Methods]Daily data on outpatient visits for allergic rhinitis,airborne pollen concentrations,atmospheric pollutants,and meteorological variables in Hohhot were collected from January 1,2023 to September 30,2024.Generalized additive models were used to quantitatively analyze the exposure-response relationship between pollen exposure levels and allergic rhinitis outpatient visits. [Results]Pollen concentrations in Hohhot showed a bimodal distribution from 2023 to 2024.Single-day lag effect analysis revealed that the effect on allergic rhinitis outpatient visits peaked at lag 2 days across different pollen seasons.Per interquartile range(IQR)increase in pollen concentration,the excess risk(ER)of allergic rhinitis outpatient visits was most significant during the autumn season,with an ER of 10.52%(95%CI:2.97%,18.62%).Cumulative lag analysis showed that the effect values for the entire study period and the spring season both peaked at lag 0-1 days,while the ER for the autumn season peaked at lag 0-2 days,and for the low-concentration period at lag 0-3 days.The corresponding peak ERs were 3.43%(95%CI:2.44%,4.42%),2.80%(95%CI:0.60%,5.06%),7.55%(95%CI:3.34%,11.93%),and 4.22%(95%CI:1.96%,6.52%),respectively.Furthermore,the cumulative lag effect decreased as the lag duration increased.The exposure-response relationship between pollen and outpatient visits exhibited a nonlinear pattern throughout the entire study period,characterized by a steep rise at low concentrations followed by a gentle slope at high concentrations.Conversely,it showed a linear relationship with a threshold during the spring and autumn seasons. [Conclusion]Elevated pollen exposure in Hohhot is significantly associated with an increased risk of outpatient visits for allergic rhinitis.These effects are primarily short-term and acute,exhibiting distinct season-specific patterns.The single-day lag effects consistently peak at lag 2 days.The cumulative lag effects peak at lag 0-1 days for both the entire study period and the spring season,and at lag 0-2 days for the autumn season.The lag effect declines rapidly with increasing lag duration,providing a scientific basis for establishing local pollen health alerts and implementing targeted protective measures for susceptible populations.
