[背景]血脂异常是心血管疾病的主要危险因素.残余胆固醇(RC)作为一种新型血脂指标,也被认为是心血管疾病发展的潜在风险因素.目前,关于轮班工作对血脂异常影响的研究结论尚不一致,且尤其缺乏针对轮班工作与RC之间关联的探讨. [目的]分析护士不同轮班工作特征的潜类别特点,并探讨其与护士 RC、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)的关联,为促进护士的血脂健康提供科学依据. [方法]本研究为横断面研究,数据来源于环境职业暴露与护士健康队列基线调查.综合考虑地域分布与医院等级,从队列覆盖的安徽省皖北、皖中、皖南地区分别随机选取 1所综合性医院,于 2022年 7月至 2025年 9月对符合纳入标准的在职护士开展调查.共 1 933名护士完成基线问卷,最终纳入问卷有效且血生化指标完整的 1 079名护士进行分析.采用自编的护士生殖健康调查问卷评估护士轮班工作特征(轮班、夜班频率、每天工作时长、每周工作天数、工作年限、自评工作强度).收集护士血脂指标,包括TC、TG、HDL-C、LDL-C,并计算RC.使用秩和检验比较不同人口统计学特征护士 RC的差异,运用 X2 检验比较不同人口统计学特征护士轮班类别的差异.使用 R语言 poLCA软件包进行潜类别分析(LCA)以识别轮班工作特征的潜在类型,并通过雷达图将分析结果进行可视化呈现.使用广义线性模型分析轮班类别和护士血脂指标(RC、TC、TG、HDL-C、LDL-C)连续性变量的关联,并采用二元 logistic回归分析轮班类别与血脂指标二分类变量的关联. [结果]护士轮班类别分为 3种:轮班且高工作强度组(38.1%)、低工作强度且短工时组(32.0%)、常规工时且长工作年限组(29.9%).以低工作强度且短工时组为参照进行广义线性模型分析,结果显示,常规工时且长工作年限组与 RC(β=0.07,95%CI:0.01~0.14)、LDL-C(β=0.13,95%CI:0.01~0.24)、TC(β=0.22,95%CI:0.08~0.36)浓度升高相关,轮班且高工作强度组与 RC浓度升高相关(β=0.12,95%CI:0.07~0.18).根据年龄分层并调整协变量后,结果显示,常规工时且长工作年限组与 RC的关联主要体现在≤33岁的护士中,与 LDL-C的关联主要体现在≥34岁的护士中;轮班且高工作强度组与 RC的关联主要体现在≤33岁的护士中.二元 logistic回归分析结果显示,调整协变量后,与参照组相比,常规工时且长工作年限组中,RC≥1.07 mmol·L-1(OR=2.12,95%CI:1.43~3.13)、LDL-C≥3.40 mmol·L-1(OR=1.76,95%CI:1.04~2.98)和 TC≥5.20 mmol·L-1(OR=2.04,95%CI:1.38~3.02)风险均增加;轮班且高工作强度组仅RC≥1.07 mmol·L-1 风险升高(OR=1.96,95%CI:1.36~2.84). [结论]护士不同轮班工作特征存在明显的分类特征,并与护士 RC、LDL-C、TC浓度关系密切.管理者应重视其对护士血脂健康的影响,合理制定工作时间表,以降低轮班工作对血脂健康的负面影响.

[Background]Dyslipidemia is a major risk factor for cardiovascular disease(CVD).Remnant cholesterol(RC),an emerging lipid biomark-er,has also been recognized as a potential risk factor for CVD development.Currently,epidemiological findings on the impact of shift work on dyslipidemia remain inconsistent,and studies examining its association with RC are particularly limited. [Objective]To analyze latent classes of shift work characteristics among nurses and examine their associations with RC,total cholesterol(TC),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),and low-density lipoprotein cholesterol(LDL-C),thereby providing evidence for promoting lipid health in this occupational population. [Methods]This cross-sectional study utilized data from the baseline survey of the Nurses'Environmental&Occupational Exposure and Health Nexus Cohort(NEON Cohort).Considering geographical distribution and hospital grade,one general hospital was randomly selected from the cohort-covered hospitals in each of the northern,central,and southern regions of Anhui Province.From July 2022 to September 2025,a cross-sectional survey was conducted among practicing nurses who met the inclusion criteria in the selected hospitals.A total of 1 933 nurses completed the baseline questionnaire.After excluding participants with incomplete questionnaires or missing blood bio-chemical indicators,1 079 nurses were finally included in the analysis.Shift work characteristics—including shift work status,night shift frequency,daily working hours,weekly working days,years of work experience,and self-rated work intensity—were assessed using a self-designed Reproductive Health Questionnaire for Nurses.Blood lipid profiles(TC,TG,HDL-C,and LDL-C)were collected,and RC was calculated accordingly.The Wilcoxon rank-sum test and Chi-square test were used to compare RC levels and shift work patterns across demographic variables,respectively.Latent class analysis(LCA)was performed using the poLCA package in R to identify potential patterns of shift work,and the results were visualized using radar charts.Generalized linear models(GLM)and binary logistic regression were used to examine the associations between shift work categories and continuous/dichotomized lipid measures. [Results]Three latent classes of shift work characteristics were identified among nurses:the shift work and high intensity group(38.1%),the low intensity and short hours group(32.0%),and the regular hours and long tenure group(29.9%).Using the low intensity and short hours group as the reference in GLM analysis,the regular hours and long tenure group was significantly associated with elevated levels of RC(β=0.07,95%CI:0.01,0.14),LDL-C(β=0.13,95%CI:0.01,0.24),and TC(β=0.22,95%CI:0.08,0.36).The shift work and high intensity group was also associated with elevated RC levels(β=0.12,95%CI:0.07,0.18).After stratification by age and adjustment for covariates,the association between the regular hours and long tenure group and RC was primarily observed in nurses aged≤ 33  years,whereas its association with LDL-C was prominent in those aged≥ 34  years.The association between the shift work and high intensity group and RC was also predominantly observed in nurses aged≤33 years.Logistic regression showed that,compared with the reference group,the regular hours and long tenure group had significantly increased risks for RC≥1.07 mmol·L-1(OR=2.12,95%CI:1.43,3.13),LDL-C≥3.40 mmol·L-1(OR=1.76,95%CI:1.04,2.98),and TC≥5.20 mmol·L-1(OR=2.04,95%CI:1.38,3.02).The shift work and high intensity group showed a significantly increased risk of RC≥1.07 mmol·L-1(OR=1.96,95%CI:1.36,2.84). [Conclusion]Nurses'shift work characteristics exhibit distinct latent classification patterns,which are significantly associated with RC,LDL-C,and TC levels.Occupational health administrators should consider these pattern-specific impacts on nurses'lipid health and imple-ment rationally designed work schedules to mitigate the adverse lipid health effects of shift work.
