内蒙古自治区金属矿山工人氡暴露水平及健康风险评估OA
Radon exposure levels and health risk assessment among metal mine workers in Inner Mongolia Autonomous Region
[背景]氡属于天然放射性物质,其通过呼吸道进入人体,可对肺组织造成损伤.采矿过程中氡气释放会给矿工带来健康风险. [目的]了解内蒙古自治区地区金属矿山井上及井下环境中氡浓度,并评估矿工肺结节的影响因素. [方法]选取内蒙古 6座正常生产金属矿山,采用主动式测氡仪检测井上、井下工作场所环境氡浓度.使用风速仪对抽出式通风矿山不同位置的风速进行测量.使用 Spearman相关性分析方法分析风速与环境氡浓度的关系.以被动式氡个人剂量计评估 118名男性矿工两个月内氡及其子体所致个人待积有效剂量.通过低剂量 CT、染色体畸变分析、微核分析、眼晶体检查及白细胞计数评估矿工放射性健康状况,并以问卷调查收集年龄、工龄、吸烟情况、防护用品佩戴情况等基本信息.采用贝叶斯广义线性混合模型(BGLMM),以肺结节为应变量,矿山作为随机截距项,分析年龄、吸烟状况及吸烟与职业暴露后的个人待积有效剂量交互作用对肺结节风险的影响. [结果]6座矿山井下各监测点的平均氡浓度范围为 24.6~3 126.9 Bq·m-3,其中 4座超过1 000 Bq·m-3;井上各监测点的平均氡浓度范围为 5.8~778.9 Bq·m-3.风速与井下环境氡浓度呈负相关(r=-0.428,P<0.05),其中作业区风速最小,氡浓度最高.118名矿工中,14名矿工 2个月待积有效剂量超过2 mSv(井上3名,井下11名),胸部低剂量CT提示11人检出肺结节,其中井上 4人,井下 7人.BGLMM分析结果显示,年龄与肺结节风险呈正向关联(后验OR=1.121,95%CI:1.036~1.224);职业暴露后的个人待积有效剂量与吸烟仅呈微弱正向趋势,吸烟与职业暴露后的个人待积有效剂量的交互作用不具有统计学意义. [结论]内蒙古自治区部分金属矿山存在井上及井下环境氡浓度较高,部分区域通风不足等问题,部分矿工个人待积有效剂量接近或超过个人剂量限值.在矿工肺结节影响因素中,年龄是肺结节的主要风险因素.
[Background]Radon is a naturally occurring radioactive gas that can enter the human body through the respiratory tract and cause damage to lung tissue.Radon release during mining op-eration poses significant health risks to miners. [Objective]To investigate radon concentrations in surface and underground workplaces of metal mines in Inner Mongolia Autonomous Region,and to evaluate factors associated with pulmonary nodules among miners. [Methods]Six actively producing metal mines in Inner Mongolia were selected.Active radon monitors were employed to measure ambient radon concentrations in surface and underground workplaces.Anemometers were used to measure wind speeds at different locations in mines equipped with exhaust ventilation systems.Spearman correlation analysis was per-formed to examine the relationship between wind speed and ambient radon concentration.Passive radon personal dosimeters were used to estimate individual committed effective doses from radon and its progeny among 118 male miners over a two-month monitoring period.Radiological health status was assessed using low-dose CT,chromosomal aberration analysis,micronucleus assay,ocular lens ex-amination,and white blood cell counts.Information on age,work history,smoking status,and personal protective equipment usage was collected using a questionnaire.A Bayesian generalized linear mixed model(BGLMM)was fitted to examine the associations of cumulative personal dose,age,smoking status,and the smoking individual committed effective dose after occupational exposure interaction with pulmonary nodule risk,with mine included as a random intercept. [Results]The mean radon concentrations at underground monitoring points in the the six mines ranged from 24.6 to 3 126.9 Bq·m-3,with four mines exceeding 1 000 Bq·m-3.The mean radon concentrations at surface monitoring points ranged from 5.8 to 778.9 Bq·m-3.Wind speed was negatively correlated with underground radon concentration(r=-0.428,P<0.05),with the lowest wind speed and the highest radon concentration observed in the working areas.Among the 118 miners,14 had a two-month committed effective dose ex-ceeding 2 mSv(3 surface workers and 11 underground workers).Low-dose chest CT detected pulmonary nodules in 11 miners(4 surface workers and 7 underground workers).The BGLMM results showed that age was positively associated with pulmonary nodule risk(posterior OR=1.121,95%CI:1.036,1.224).Committed effective dose and smoking showed weak positive trends,whereas the smokin committed effective dose interaction was not statistically significant. [Conclusion]Some metal mines in Inner Mongolia Autonomous Region exhibit elevated radon concentrations in both surface and under-ground workplaces,and inadequate ventilation is observed in certain work areas.A proportion of miners have relatively high personal committed effective doses.Among the examined factors,age is the main factor associated with pulmonary nodules in miners.
