气温与鼻咽癌特异性死亡的关联及细颗粒物的效应修饰作用OA
Association between ambient temperature and nasopharyngeal carcinoma-specific mortality:Effect modification by PM2.5
[背景]在气候变化背景下,气温升高对人群健康的影响日益受到关注.鼻咽癌具有明显的地域聚集性,高发地区多位于气温较高区域,提示气温可能在疾病预后中发挥作用. [目的]探讨气温与鼻咽癌预后结局之间的关联,并评估不同暴露时间窗口中肿瘤分期和细颗粒物(PM2.5)的效应修饰作用. [方法]基于中山大学肿瘤防治中心建立的鼻咽癌队列,纳入2018-2020年确诊的鼻咽癌患者7 780例.气温和 PM2.5 暴露数据来源于国家青藏高原科学数据中心和中国高分辨率高质量近地表空气污染物(CHAP)数据集,通过个体居住地址匹配结局发生前 6个月、12个月及 36个月的平均气温水平.采用 Cox比例风险模型评估气温(每升高 1℃)与鼻咽癌特异性死亡、复发及远处转移的关联,并按原发肿瘤分期(T分期)、区域淋巴结分期(N分期)和 PM2.5 暴露水平进行分层分析,以评估潜在的效应修饰作用. [结果]随访期间,共发生鼻咽癌特异性死亡 897例、复发 1 722例、远处转移 1 640例.校正 PM2.5 后,气温每升高 1℃,鼻咽癌特异性死亡风险升高(6个月:HR=1.11,95%CI:1.08~1.14;12个月:HR=1.18,95%CI:1.13~1.24;36个月:HR=1.25,95%CI:1.16~1.34).分层分析显示,该关联在肿瘤 T3~T4期 N2~N3期人群中更强(以 12个月为例,T3~T4组 HR=1.19,95%CI:1.13~1.25;N2~N3组 HR=1.21,95%CI:1.14~1.27).以各时间窗口 PM2.5 暴露水平中位数(6个月:23.3 μg·m-3;12个月:21.8 μg·m-3;36个月:22.5 μg·m-3)为界进行分层后,高PM2.5 暴露组中气温与鼻咽癌特异性死亡风险之间的关联更强(P交互<0.05).未观察到气温与鼻咽癌复发、转移存在长期稳定的统计学关联,但在 6个月和 12个月时间窗口校正 PM2.5后呈现微弱正向关联(复发,6个月,HR=1.04,95%CI:1.02~1.06;12个月,HR=1.06,95%CI:1.03~1.10;转移,6个月,HR=1.02,95%CI:1.00~1.05;12个月,HR=1.05,95%CI:1.01~1.10). [结论]环境气温与鼻咽癌特异性死亡风险升高相关,且在肿瘤晚期人群中作用更为明显.环境气温可能影响鼻咽癌预后,并与空气污染存在协同作用.
[Background]Climate change has raised concerns regarding the health impacts of rising temper-atures.Nasopharyngeal carcinoma(NPC)is geographically clustered in warm regions,implying a potential prognostic role of ambient temperature. [Objective]To investigate the association between ambient temperature and NPC prognosis,and to evaluate effect modification by tumor stage and fine particulate matter(PM2.5)across dif-ferent exposure windows. [Methods]A total of 7 780 NPC patients diagnosed between 2018 and 2020 were enrolled from a cohort at the Sun Yat-sen University Cancer Center.Temperature and PM2.5 data were obtained from public datasets of the National Tibetan Plateau Data Center and ChinaHighAirPollutants(CHAP)and matched to patients'residential addresses to calculate average exposure 6,12,and 36 months prior to the outcome.Cox proportional hazards models were used to assess the associations of temperature(per 1℃increase)with NPC-specific death,recurrence,and metastasis.Stratified analyses were conducted according to primary tumor stage(T stage),re-gional lymph node stage(N stage),and PM2.5 levels to evaluate potential effect modification. [Results]During follow-up,897 NPC-specific deaths,1 722 recurrences,and 1 640 metastases were recorded.Each 1 ℃ increase in ambient temperature was significantly associated with an increased risk of NPC-specific death across all exposure windows(6 month:HR=1.11,95%CI:1.08,1.14;12 month:HR=1.18,95%CI:1.13,1.24;36 month:HR=1.25,95%CI:1.16,1.34).Stratified analysis showed that this as-sociation was more pronounced among patients with tumor stages T3-T4 and lymph node stages N2-N3(for example,at 12 months,the HR for the T3-T4 group was 1.19,95%CI:1.13,1.25;for the N2-N3 group,the HR was 1.21,95%CI:1.14,1.27).In addition,the association between temperature and NPC-specific death was stronger in the high PM2.5 group(defined using median PM2.5 concentrations:23.3,21.8,and 22.5 μg·m-3 for the 6-,12-,and 36-month windows,respectively),with significant interaction effects observed(Pinteraction<0.05).Temperature showed no long-term association with recurrence or metastasis,while weak positive associations(recurrence:6 months,HR=1.04,95%CI:1.02,1.06;12 months,HR=1.06,95%CI:1.03,1.10;metastasis:6 months,HR=1.02,95%CI:1.00,1.05;12 months,HR=1.05,95%CI:1.01,1.10).were observed for the 6-and 12-month windows after adjusting for PM2.5. [Conclusion]Higher ambient temperature is significantly associated with an increased risk of NPC-specific death,especially among patients with advanced T and N stages.Ambient temperature may negatively influence NPC prognosis and act synergistically with PM2.5 exposure.
