The relationship between serum FOXO1,MFG-E8 and periodontal indicators in patients with periodontitis and their interactive influence on prognosis

目的:探讨血清叉头框蛋白 O1(forkhead box protein O1,FOXO1)、乳脂肪球表皮生长因子 8(milk fat globule-epidermal growth factor 8,MFG-E8)水平与牙周炎患者牙周临床指标的相关性,分析二者对牙周炎非手术治疗短期疗效的预测价值及交互影响.方法:选取 2023 年 10 月~2025 年 10 月于本院就诊的 118 例慢性牙周炎患者作为研究组,另选 60 例同期健康体检者作为对照组.采用酶联免疫吸附试验检测血清 FOXO1、MFG-E8 水平.记录研究组患者全口平均探诊深度(probing depth,PD)、临床附着丧失(clinical attachment loss,CAL)、龈沟出血指数(sulcus bleeding index,SBI)及菌斑指数(plaque index,PLI).牙周炎患者接受标准化非手术牙周治疗,治疗后 3 个月根据疗效情况分为疗效良好组(75 例)与疗效不良组(43 例).分析血清 FOXO1、MFG-E8 水平与牙周指标的相关性;采用多因素 Logistic 回归分析影响短期疗效的因素;采用受试者工作特征曲线(receiver operating characteristic curve,ROC)评估单项及联合指标的预测效能,Bootstrap 自抽样法进行内部验证;通过交互作用模型分析 FOXO1 与 MFG-E8 对短期疗效的交互作用.结果:研究组血清 FOXO1 水平显著高于对照组,MFG-E8 水平显著低于对照组(P<0.05).疗效不良组基线 PD、CAL 及血清 FOXO1 水平显著高于疗效良好组,MFG-E8 水平显著低于疗效良好组(P<0.05).FOXO1 与 PD、CAL、SBI、PLI 均呈显著正相关,MFG-E8 与上述指标均呈显著负相关(P<0.05).多因素Logistic 回归分析校正基线 PD、CAL 后,高 FOXO1 水平是牙周炎疗效不良的危险因素(OR=2.449,95%CI 1.260~4.759),高 MFG-E8 水平是保护因素(OR=0.861,95%CI 0.782~0.948).ROC 曲线分析显示,血清 FOXO1、MFG-E8预测疗效不良的曲线下面积(area under the curve,AUC)分别为0.821、0.803,二者联合检测 AUC 显著提升至0.910,灵敏度 81.40%,特异度 88.00%,内部验证显示校正后曲线下面积为 0.892,校准曲线一致性良好.交互作用分析显示,当患者同时呈现高 FOXO1 水平与低 MFG-E8 水平时,其疗效不良风险显著增加(OR=8.509,95%CI 2.522~28.708),二者存在正向交互作用(RERI=4.032,AP=47.39%,SI=2.160).结论:血清 FOXO1 和 MFG-E8 水平与牙周炎病情严重程度密切相关,是影响患者非手术治疗疗效的因素.二者联合检测对疗效不良具有较高的预测价值,且对疗效存在正向交互影响,可能协同加剧牙周组织破坏,为牙周炎的精准风险分层提供了新的血清学标志物.

Objective:To investigate the correlation between serum forkhead box protein O1(FOXO1)and milk fat globule epidermal growth factor 8(MFG-E8)levels and periodontal clinical indicators in patients with periodontitis,and to e-valuate their predictive value and interactive effects on short-term outcomes of non-surgical treatment for periodontitis.Meth-ods:A total of 118 patients with chronic periodontitis who visited our hospital from October 2023 to October 2025 were select-ed as the study group,and 60 healthy individuals undergoing physical examinations during the same period were selected as the control group.Serum levels of FOXO1 and MFG-E8 were measured using enzyme-linked immunosorbent assay.The mean probing depth(PD),clinical attachment loss(CAL),sulcus bleeding index(SBI)and plaque index(PLI)for patients in the study group were recorded.Patients with periodontitis received standardized non-surgical periodontal treatment and were di-vided into a good outcome group(75 cases)and a poor outcome group(43 cases)based on treatment efficacy at 3 months post-treatment.Correlations between serum FOXO1,MFG-E8 levels and periodontal indicators were analyzed.Multivariate lo-gistic regression was used to identify factors influencing short-term outcomes.Receiver operating characteristic(ROC)curves were used to evaluate the predictive performance of individual and combined indicators,with internal validation performed u-sing the Bootstrap resampling method.Interactive effects of FOXO1 and MFG-E8 on short-term outcomes were analyzed using an interaction model.Results:Serum FOXO1 levels were significantly higher and MFG-E8 levels significantly lower in the study group than in the control group(P<0.05).Baseline PD,CAL,and serum FOXO1 levels were significantly higher,while MFG-E8 levels were significantly lower in the poor outcome group compared with the good outcome group(P<0.05).FOXO1 was significantly positively correlated with PD,CAL,SBI,and PLI,while MFG-E8 was significantly negatively correlated with these indicators(P<0.05).Multivariate logistic regression adjusting for baseline PD and CAL showed that high FOXO1 level was a risk factor for poor outcome(OR=2.449,95%CI 1.260~4.759),and high MFG-E8 level was a protective factor(OR=0.861,95%CI 0.782~0.948).ROC curve analysis showed that the area under the curve(AUC)for predicting poor outcome was 0.821 for serum FOXO1 and 0.803 for MFG-E8,and the AUC of combined detection was significantly increased to 0.910,with a sensitivity of 81.40%and specificity of 88.00%.Internal validation showed a corrected AUC of 0.892,and the calibration curve demonstrated good agreement.Interaction analysis revealed that patients with both high FOXO1 and low MFG-E8 levels had a significantly increased risk of poor outcome(OR=8.509,95%CI 2.522~28.708),indicating a posi-tive interaction(RERI=4.032,AP=47.39%,SI=2.160).Conclusion:Serum FOXO1 and MFG-E8 levels are closely asso-ciated with the severity of periodontitis and are factors influencing the short-term outcome of non-surgical treatment.Combined detection of these two markers has high predictive value for poor treatment outcome,and they exhibit a positive interactive effect,potentially synergistically exacerbating periodontal tissue destruction,providing new serological markers for precise risk stratification of periodontitis.
