CAD/CAM全瓷髓腔固位冠修复根管治疗后磨牙牙体缺损的回顾性研究OA
Retrospective study of CAD/CAM all-ceramic endocrowns for restoring tooth defect in molars after root canal treatment
目的:探讨 CAD/CAM 全瓷髓腔固位冠修复根管治疗后磨牙牙体缺损临床应用效果及失败原因.方法:回顾性分析2018 年2 月～2021 年2 月于本院收治纳入并随访到的276 颗(上颌磨牙112 颗/下颌磨牙164 颗)患牙为研究对象,采用世界牙科联盟(FDI,World Dental Federation)评价标准对髓腔固位冠修复临床效果评价,评价5～8 年(平均 6.5 年)髓腔固位冠修复成功率、存留率与失败率,并记录患者满意度,分析失败原因.结果:276 颗患牙髓腔固位冠修复成功率为 89.49%,存留率为 93.12%,失败率为 10.51%.患者对髓腔固位冠解剖外形、颜色及舒适度的满意率分别达到 98.19%、98.55%和 98.55%.上颌磨牙组与下颌磨牙组比较差异无统计学意义(P>0.05).结论:CAD/CAM 全瓷髓腔固位冠修复根管治疗后磨牙牙体缺损的临床效果良好,患者满意度高,但需严格把握适应证并规范临床操作以降低失败风险.
Objective:To explore the clinical effects and failure reasons of CAD/CAM all-ceramic endocrowns for molars after root canal treatment.Methods:A retrospective study was conducted on follow-up 276 teeth(112 maxillary molars and 164 mandibular molars)treated in author's hospital from February 2018 to February 2021.The clinical outcomes of the endocrowns were evaluated using the World Dental Federation(FDI)criteria.The success rate,survival rate,and failure rate of endocrowns over 5～8 years(mean 6.5 years)were studied and analyzed,along with patient satisfaction and failure causes.Results:The success rate of endocrowns was 89.49%,the survival rate was 93.12%,and the failure rate was 10.51%.Pa-tient satisfaction rates for the shape,color,and comfort of endocrowns reached 98.19%,98.55%,and 98.55%,respectively.No statistically significant difference was observed between the maxillary and mandibular molar groups(P>0.05).Conclu-sion:CAD/CAM all-ceramic endocrowns demonstrate favorable clinical outcomes and high patient satisfaction for molars after root canal treatment.However,strict adherence to indications and standardized clinical procedures are essential to reduce fail-ure risks.
