三种加力方式下近中移动下颌磨牙的有限元分析OA
Finite element analysis of mesial molar movement under three force application methods
目的:采用三维有限元方法,比较下颌第一磨牙在固定矫治 3 种加力方式下近中移动时的生物力学效应.方法:构建包含下颌第一磨牙(双根)、牙周膜、牙槽骨及矫治器的三维有限元模型,设置①颊侧加力(1.5 N)、②舌侧加力(1.5 N)、③颊舌双侧加力(各 0.75 N)3 种方式,模拟下颌第一磨牙近中移动,分析其位移趋势与牙周膜主应力分布特征.结果:3 种加力方式下,下颌第一磨牙均呈近中倾斜移动伴近中侧压低、远中侧(牙合)向移动;颊侧加力(1.5 N)与舌侧加力(1.5 N)均伴一定程度的扭转,牙周膜主应力分布不均匀;颊舌双侧加力(各 0.75 N)时扭转明显减小,牙周膜主应力分布更均匀;倾斜度大小依次为颊侧加力(1.5 N)0.0392°>舌侧加力(1.5 N)0.0386°>颊舌双侧加力(各 0.75 N)0.0374°.结论:颊舌双侧加力(各 0.75 N)可有效抵消颊舌向不利力矩,实现更可控的位移、更小的倾斜度与更均匀的牙周膜应力分布,接近整体移动,更有利于牙周组织健康.
Objective:To compare the biomechanical effects of three orthodontic force application modes on the me-sial movement of the mandibular first molar using three-dimensional finite element analysis.Methods:A three-dimensional fi-nite element model comprising the mandibular first molar(two-rooted),periodontal ligament,alveolar bone,and orthodontic appliance was constructed.Three loading conditions were simulated to induce mesial movement:①buccal force(1.5 N),②lingual force(1.5 N),③bilateral buccal-lingual forces(0.75 N each).The displacement tendency of the tooth and the prin-cipal stress distribution in the periodontal ligament were analyzed for each condition.Results:Under all three loading modes,the mandibular first molar exhibited mesial tipping movement,characterized by distal extrusion and mesial intrusion.Both uni-lateral buccal(1.5 N)and lingual(1.5 N)force applications induced a certain degree of undesirable rotation,accompanied by uneven principal stress distribution in the periodontal ligament.In contrast,bilateral force application(0.75 N each)substan-tially reduced rotation and yielded a more uniform stress pattern.The magnitude of tipping angulation was ranked as follows:buccal force(1.5 N)at 0.0392°>lingual force(1.5 N)at 0.0386°>bilateral forces(0.75 N each)at 0.0374°.Conclusion:Bilateral buccal-lingual force application(0.75 N each)effectively counteracts unfavorable buccolingual moments,enabling more controlled tooth displacement,reduced tipping,and a more homogeneous stress distribution within the periodontal liga-ment.This loading strategy approaches bodily movement and is more favorable for periodontal tissue health.
