Er:YAG激光工作尖插入深度对根管内氢氧化钙清除效果的影响OA
Effect of Er:YAG laser on the removal of calcium hydroxide from root canals at different insertion depths of the tip
目的:探讨 Er:YAG 激光工作尖插入髓腔深度对根管内氢氧化钙 Ca(OH)2 清除效果的影响,为临床优化激光活化冲洗技术提供实验依据.方法:选取 30 颗符合纳入标准的单根管离体前磨牙,统一截冠至长度均为18 mm,常规根管预备后封入Ca(OH)2 糊剂,于37℃恒温恒湿环境储存7 d.将样本随机分为3 组(n=10),采用Er:YAG 激光 SWEEPS 模式行活化冲洗,工作尖插入深度分别为 A 组 4 mm、B 组 7 mm、C 组 10 mm.分别在 Ca(OH)2清除前和清除后使用锥形束 CT(CBCT)对标本进行扫描并将 DICOM 数据导入 Mimics 软件进行三维重建,计算出根管内 Ca(OH)2 清除前、后的体积与清除率.利用扫描电镜(SEM)观察根尖区根管壁 Ca(OH)2 残留情况并按标准进行清洁度评分.结果:CBCT 结果显示,3 组根管内均可见 Ca(OH)2 残留,主要集中于根尖区,呈高密度阻射影;3组Ca(OH)2 清除率均达97%以上,其中C 组>B 组>A 组,A 组和C 组存在统计学差异(P<0.05),其余无明显统计学差异.扫描电镜显示 3 组根管内壁均存在不同程度 Ca(OH)2 残留,其中 C 组表面 Ca(OH)2 颗粒最少,牙本质小管暴露最充分;半定量评分显示,C 组清洁度显著优于 B 和 A 组(P<0.05),而 A 组和 B 组间无统计学差异.结论:Er:YAG 激光工作尖插入深度在根管内 Ca(OH)2 封药的清除方面具有"近端优势"效应,插入越深,清除效果越好.
Objective:To investigate the effect of Er:YAG laser tip insertion depth on the removal efficacy of intra-canal calcium hydroxide Ca(OH)2 dressing,and to provide experimental evidence for optimizing clinical laser-activated irri-gation technology.Methods:A total of 30 isolated single premolars meeting the inclusion criteria were selected,and all crowns were sectioned to a uniform length of 18 mm.After routine root canal preparation,the root canals were filled with Ca(OH)2 paste,stored in a constant temperature and humidity environment at 37℃for 7 days.The samples were randomly divided into three groups(n=10 each),and root canal laser-activated irrigation was performed using an Er:YAG laser in SWEEPS mode,with the laser tip insertion depths set at 4 mm(Group A),7 mm(Group B),and 10 mm(Group C),respec-tively.Cone-beam computed tomography(CBCT)scans were performed on all samples before and after Ca(OH)2 removal.The DICOM data were imported into Mimics software for three-dimensional reconstruction to calculate the intracanal Ca(OH)2 volume before removal(V1)and after removal(V2),the removal rate was calculated.Scanning electron microscopy(SEM)was used to observe the residual Ca(OH)2 on the root canal walls in the apical region of each group,and the cleanliness was scored according to standard criteria.Results:CBCT results showed residual Ca(OH)2 in the root canals of all three groups,mainly concentrated in the apical region,presenting as hyperdense radiopaque images.The Ca(OH)2 removal rates of Groups A,B,and C all exceeded 97%,a statistically significant difference was observed between group A and group C(P<0.05),while no significant differences were found among the other comparisons.SEM observations demonstrated varying degrees of Ca(OH)2 residue on the inner root canal walls of the three groups;Group C exhibited the fewest Ca(OH)2 particles on the surface and the most fully exposed dentinal tubules.Semi-quantitative scoring results indicated that the cleanliness of Group C was significantly superior to that of Groups B and A(P<0.05),while no significant statistical difference was found between Group A and Group B.Conclusion:The insertion depth of the Er:YAG laser tip exerts a"proximal advantage"effect on the removal of intracanal Ca(OH)2 dressing,with a deeper insertion corresponding to a better removal efficacy.
