iRoot SP糊剂单尖法与AH-Plus糊剂热牙胶垂直加压充填在根尖周病根管治疗中的疗效比较OA
Comparison on the curative effect of iRoot SP paste single-tip method and AH-Plus paste warm gutta-percha vertical compression in root canal filling for periapical diseases
目的:探讨 iRoot SP 糊剂单尖法充填、AH-Plus 糊剂热牙胶垂直加压充填在根尖周病根管充填中的疗效.方法:选取 2023 年 6 月～2025 年 9 月在本院根尖周病根管治疗的患者 124 例,随机分为 iRoot SP 组(62 例,71颗)和 AH-Plus 组(62 例,74 颗),iRoot SP 组采用 iRoot SP 糊剂单尖法充填,AH-Plus 组采用 AH-Plus 糊剂热牙胶垂直加压充填,治疗后均随访 3 个月.比较两组患者疗效、叩痛及咬合痛发生情况、牙周情况、根充密合程度、龈沟液骨保护素(osteoprotegerin,OPG)、核因子-κB 受体活化因子配体(receptor activator of nuclear factor-kappa B ligand,RANKL).结果:随访 3 个月后,iRoot SP 组和 AH-Plus 组疗效比较无显著差异;iRoot SP 组叩痛及咬合痛发生率低于AH-Plus 组(P<0.05).随访3 个月后,iRoot SP 组菌斑指数、龈沟出血指数低于AH-Plus 组(P<0.05);iRoot SP 组OPG 水平高于 AH-Plus 组、RANKL 水平低于 AH-Plus 组(P<0.05).结论:iRoot SP 糊剂单尖法充填、AH-Plus 糊剂热牙胶垂直加压充填治疗根尖周病根管充填中的疗效均较好,但 iRoot SP 糊剂单尖法充填能更有效减轻患者叩痛及咬合痛,改善患者牙周情况和 OPG、RANKL 水平.
Objective:To explore the curative effect of iRoot SP paste single-tip method and AH-Plus paste warm gutta-percha vertical compression in root canal filling for periapical diseases.Methods:A total of 124 patients undergoing root canal filling for periapical diseases in the hospital were randomly divided into iRoot SP group(62 cases,71 teeth,iRoot SP paste single-tip method)and the AH-Plus group(62 cases,74 teeth,AH-Plus paste warm gutta-percha vertical compression)between June 2023 and September 2025.All patients were followed up for 3 months after treatment.The curative effect,occur-rence of percussion pain and occlusal pain,periodontal condition,root filling tightness,osteoprotegerin(OPG)in gingival crev-icular fluid and receptor activator of nuclear factor-kappa B ligand(RANKL)in the two groups were compared.Results:After 3 months of follow-up,there was no difference in curative effect between the two groups(P>0.05),the incidence of percus-sion pain and occlusal pain in iRoot SP group was lower than that in AH-Plus group(P<0.05).After 3 months of follow-up,plaque index and sulcus bleeding index in iRoot SP group were lower than those in AH-Plus group(P<0.05).OPG level in iRoot SP group was higher than that in AH-Plus group,while RANKL level was lower than that in AH-Plus group(P<0.05).Conclusion:The curative effect of both iRoot SP paste single-tip method and AH-Plus paste warm gutta-percha vertical com-pression for root canal filling is good in periapical diseases.However,the former can more effectively relieve percussion pain and occlusal pain,improve periodontal condition,OPG and RANKL levels.
