童年饥荒暴露与中老年肌少症的关联研究OA
Association between childhood famine exposure and sarcopenia in middle-aged and elderly individuals
目的 探讨童年饥荒暴露与中老年肌少症的关联.方法 基于中国健康与养老追踪调查 2011 年基线和 2015 年随访数据,根据出生日期(饥荒暴露的替代变量)将参与者分为非暴露组、胎儿期暴露组、学龄前期暴露组和学龄期暴露组,依据亚洲肌少症工作组 2019 年共识对肌少症及其组分(低肌肉质量、低肌肉力量、躯体功能低下)进行定义.运用多因素 Logistic 回归分析儿童期不同阶段饥荒暴露与肌少症及其组分的患病率、发病率之间的关联.结果 基线分析纳入 6 988 人,肌少症患病率随暴露组次序递增(2.41%,3.72%,6.60%,10.12%).调整混杂因素后,学龄前期和学龄期饥荒暴露是肌少症患病的危险因素(OR=3.38,95%CI:2.16~5.28;OR=5.65,95%CI:3.65~8.74),也是低肌肉力量、躯体功能低下和低肌肉质量的危险因素.随访分析纳入4 781 人,学龄前期和学龄期饥荒暴露使肌少症发病风险分别增至 5.99 倍(95%CI:1.81~19.89)和 9.84 倍(95%CI:3.02~32.09),并增加各组分发病风险.结论 学龄前期和学龄期饥荒暴露显著增加中老年肌少症风险,应重视儿童期营养干预.
Objective To investigate the association between childhood famine exposure and sarcopenia in middle-aged and elderly individuals.Methods Based on the baseline data in 2011 and follow-up data in 2015 from the China Health and Retirement Longitudinal Study(CHARLS),participants were divided into non-exposed group,fetal exposure group,preschool exposure group,and school-age exposure group according to date of birth,which served as a surrogate indicator of famine exposure.Sarcopenia and its components,including low muscle mass,low muscle strength,and poor physical function,were defined in accordance with the 2019 consensus of the Asian Working Group for Sarcopenia.Multivariate Logistic regression models were adopted to analyze the relationships of famine exposure at different childhood stages with the prevalence and incidence of sarcopenia as well as its components.Results A total of 6 988 participants were enrolled in the baseline analysis.The prevalence of sarcopenia increased gradually across the exposure groups,being 2.41%,3.72%,6.60%,and 10.12%,respectively.After adjusting for confounding factors,famine exposure in preschool and school-age were risk factors for sarcopenia prevalence(OR=3.38,95%CI:2.16 to 5.28;OR=5.65,95%CI:3.65 to 8.74),and also increased the risks of low muscle strength,poor physical function,and low muscle mass.In the follow-up analysis involving 4 781 subjects,preschool and school-age famine exposure elevated the risk of incident sarcopenia to 5.99-fold(95%CI:1.81 to 19.89)and 9.84-fold(95%CI:3.02 to 32.09)respectively,and raised the onset risks of developing all sarcopenia components.Conclusion Famine exposure during preschool and school-age periods significantly increases the risk of sarcopenia in middle-aged and elderly individuals.Early nutritional intervention in childhood should be emphasized.
