目的 探讨微小核糖核酸-221(miR-221)、miR-802在前列腺癌术后复发中的交互作用,及二者单独、联合预测术后复发的效能.方法 回顾性选取2018年1月-2023年5月新乡市中心医院收治的105例前列腺癌患者,均行根治术治疗.治疗结束后随访至2025年10月31日,中位随访时间26.5个月.比较不同复发情况患者癌组织miR-221、miR-802水平.根据miR-221、miR-802水平将患者分为4组(Q1-Q4组),比较4组临床资料及术后复发率,采用Cox回归分析癌组织miR-221、miR-802水平与术后复发的关系及对术后复发的交互作用,分析miR-221、miR-802水平预测前列腺癌术后复发的效能.结果 与未复发患者比较,复发患者癌组织miR-221水平升高,miR-802水平降低(P<0.05).4组患者肿瘤分期、Gleason评分、前列腺特异抗原(PSA)水平、淋巴结转移、淋巴结清扫范围、术后放疗、术后激素治疗情况比较,差异有统计学意义(P<0.05);Q1-Q4组术后复发率分别为9.7%(3/31)、36.0%(9/25)、37.5%(9/24)、68.2%(15/22),呈逐渐升高趋势(P<0.05).Cox回归分析显示,miR-221、miR-802是前列腺癌术后复发的独立影响因素(P<0.05),且不存在多重共线性(方差膨胀因子<10);拟合优度检验结果提示该模型的拟合度良好.miR-221、miR-802均为前列腺癌术后复发的独立影响因素,且高水平miR-221与低水平miR-802在前列腺癌术后复发中呈正向交互作用(P<0.05);癌组织miR-221和miR-802水平联合预测术后复发的AUC高于任一指标单独预测(P<0.05);Bootstrap重抽样法(重复采样1000次)内部验证结果显示,miR-221、miR-802两者联合预测的一致性指数与原始的AUC接近.校正曲线显示,两者联合预测的复发概率与实际观察到的复发概率具有较高的一致性.结论 miR-221、miR-802水平对前列腺癌患者术后复发有交互作用;二者联合可为临床预测前列腺癌术后复发提供一定参考.

Objective To explore the interaction between microRNA-221(miR-221)and miR-802 in postoperative recurrence of prostate cancer,as well as the predictive efficacy of the two indicators alone and in combination for postoperative recurrence.Methods A total of 105 patients with prostate cancer who underwent radical prostatectomy at Xinxiang Central Hospital from January 2018 to May 2023 were retrospectively enrolled.All patients were followed up until October 31,2025,with a median follow-up time of 26.5 months.The expression levels of miR-221 and miR-802 in cancer tissues were compared between patients with and without recurrence.According to the levels of miR-221 and miR-802,the patients were divided into four groups:Q1-Q4.Clinical data and postoperative recurrence rates were compared among the four groups.Cox regression analysis was used to analyze the relationship between the levels of miR-221 and miR-802 in cancer tissues and postoperative recurrence,as well as their interaction on postoperative recurrence.The efficacy of miR-221 and miR-802 levels in predicting postoperative recurrence of prostate cancer was further evaluated.Results Compared with patients without recurrence,patients with recurrence had significantly higher miR-221 levels and lower miR-802 levels in cancer tissues(P<0.05).There were statistically significant differences in tumor stage,Gleason score,prostate-specific antigen(PSA)level,lymph node metastasis,extent of lymph node dissection,postoperative radiotherapy,and postoperative hormone therapy among the four groups(P<0.05).The postoperative recurrence rates in Q1 to Q4 groups were 9.7%(3/31),36.0%(9/25),37.5%(9/24)and 68.2%(15/22),respectively,showing a gradually increasing trend(P<0.05).Cox regression analysis showed that miR-221 and miR-802 were independent influencing factors for postoperative recurrence of prostate cancer(P<0.05),and no multicollinearity was observed(variance inflation factor<10).The goodness-of-fit test indicated that the model exhibited good fitness.Both miR-221 and miR-802 were independent influencing factors for postoperative recurrence of prostate cancer,and high miR-221 level combined with low miR-802 level exerted a positive interaction on postoperative recurrence of prostate cancer(P<0.05).The area under the ROC curve(AUC)of the combination of miR-221 and miR-802 in cancer tissues for predicting postoperative recurrence was significantly higher than that of either indicator alone(P<0.05).Internal validation results using the Bootstrap resampling method(1000 replications)showed that the concordance index(C-index)of the combined prediction of miR-221 and miR-802 was close to the original AUC.The calibration curve demonstrated that the recurrence probability predicted by the combination of the two indicators was highly consistent with the actually observed recurrence probability.Conclusions The expression levels of miR-221 and miR-802 in prostate cancer tissues exhibit an interaction on postoperative recurrence,and the combination of the two indicators can provide a reference for clinical prediction of postoperative recurrence in prostate cancer.
