Predictive value of triglyceride-glucose-body mass index combined with glycated hemoglobin for in-stent restenosis in coronary heart disease patients after percutaneous coronary intervention

目的 探讨甘油三酯葡萄糖体重指数(TyG-BMI)联合糖化血红蛋白(HbA1c)对接受经皮冠状动脉介入治疗(PCI)的冠心病患者术后支架内再狭窄(ISR)的预测价值.方法 收集2022年1月1日-2024年12月31日在兰州大学第一医院心内科诊断为冠心病并成功行药物洗脱支架(DES)植入术,且术后9~12个月复查冠状动脉造影(CAG)的1908例患者的临床资料进行回顾性分析.根据复查CAG结果,将发生ISR的390例作为ISR组,未发生ISR的1518例患者作为NISR组.采用倾向性评分匹配(PSM)以1:2的比例进行匹配,获得匹配后患者共848例,其中ISR组310例,NISR组538例.采用单因素和多因素logistic回归分析ISR的危险因素,并在多因素模型中纳入两者的乘积项(TyG-BMI×HbA1c)进行交互作用分析.采用多变量校正的限制性立方样条(RCS)评估TyG-BMI和HbA1c与ISR之间的剂量-反应关系,并通过阈值效应分析确定潜在的拐点.绘制受试者操作特征(ROC)曲线,通过计算曲线下面积(AUC)评估TyG-BMI和HbA1c对ISR的预测价值,并使用DeLong检验比较单一指标及联合指标间AUC的差异.结果 经过PSM后,ISR组的TyG-BMI及HbA1c水平均明显高于NISR组(P<0.001).单因素logistic回归分析提示TyG-BMI及HbA1c均与ISR相关(P<0.001);多因素logistic回归分析结果进一步显示,TyG-BMI(OR=1.03,95%CI 1.02~1.04)及HbA1c(OR=1.75,95%CI 1.49~2.07)是ISR的独立危险因素(P<0.001).ROC曲线及DeLong检验分析显示,单一指标中TyG-BMI的预测效能最佳(AUC=0.770,95%CI 0.736~0.804),优于TyG(AUC=0.725)和HbA1c(AUC=0.742),而TyG-BMI与HbA1c 联合预测ISR的效能明显高于单一指标(联合AUC=0.810,95%CI 0.779~0.841,P<0.001).多变量校正RCS结果显示,TyG-BMI与ISR之间存在线性正相关(P非线性=0.431),HbA1c与ISR之间存在非线性关系(P非线性<0.001).结论 TyG-BMI及HbA1c为冠心病患者PCI术后ISR的独立危险因素,两者对ISR均有较高的预测价值,且两者联合的预测效能更优.

Objective To investigate the predictive value of triglyceride-glucose-body mass index(TyG-BMI)combined with glycated hemoglobin(HbA1c)for in-stent restenosis(ISR)in patients with coronary heart disease(CHD)following percutaneous coronary intervention(PCI).Methods A retrospective analysis was conducted on the clinical data of 1908 patients diagnosed with CHD who underwent successful drug-eluting stent(DES)implantation at the Department of Cardiology,the First Hospital of Lanzhou University between January 1,2022,and December 31,2024,and received follow-up coronary angiography(CAG)9-12 months after surgery.According to the follow-up CAG findings,390 patients who developed ISR were assigned to ISR group,and 1518 patients without ISR to NISR group.Propensity score matching(PSM)was performed at a 1:2 ratio,yielding 848 matched patients,including 310 cases in ISR group and 538 cases in NISR group.Univariate and multivariate logistic regression analyses were performed to identify risk factors for ISR.The product term of TyG-BMI and HbA1c(TyG-BMI×HbA1c)was incorporated into the multivariate model to analyze their interaction effect.Multivariable-adjusted restricted cubic spline(RCS)analysis was conducted to evaluate the dose-response relationships of TyG-BMI and HbA1c with ISR risk,and threshold effect analysis was performed to identify potential inflection points.Receiver operating characteristic(ROC)curves were plotted,and the area under the curve(AUC)was calculated to assess the predictive efficacy of TyG-BMI and HbA1c for ISR.The DeLong test was applied to compare the differences in AUC between individual and combined indicators.Results After PSM,the levels of TyG-BMI and HbA1c in ISR group were significantly higher than those in NISR group(P<0.001).Univariate logistic regression analysis indicated that both TyG-BMI and HbA1c were correlated with ISR(P<0.001).Multivariate logistic regression analysis further verified that TyG-BMI(OR=1.03,95%CI 1.02-1.04)and HbA1c(OR=1.75,95%CI 1.49-2.07)were independent risk factors for ISR(P<0.001).ROC curve analysis and DeLong test results demonstrated that among the individual indicators,TyG-BMI had the best predictive performance for ISR(AUC=0.770,95%CI 0.736-0.804),superior to TyG(AUC=0.725)and HbA1c(AUC=0.742).The combined detection of TyG-BMI and HbA1c presented significantly higher predictive performance than any single indicator(combined AUC=0.810,95%CI 0.779-0.841,P<0.001).Multivariable-adjusted RCS analysis indicated a positive linear correlation between TyG-BMI and ISR(Pnon-linearity=0.431),while a non-linear association was observed between HbA1c and ISR(Pnon-linearity<0.001).Conclusion TyG-BMI and HbA1c are independent risk factors for ISR in CHD patients after PCI.Both have high predictive value for ISR,and their combination yields superior predictive performance.
