目的 基于胆固醇-高密度脂蛋白-葡萄糖(CHG)指数和临床指标构建列线图模型预测军事飞行人员5年高血压发病风险,为其早期识别与干预提供预测工具.方法 回顾性收集2016年1月-2020年6月于空军特色医学中心进行体检或疾病诊治的血压正常的1051名男性军事飞行人员,随机分为训练集(n=736)与验证集(n=315),并随访至2025年7月30日.采用Kaplan-Meier法、多因素Cox比例风险模型分析CHG指数水平和其他临床指标对高血压发病的影响,并构建包含CHG指数的列线图预测模型.采用受试者操作特征(ROC)曲线、校准曲线和决策曲线评估模型的区分度、校准能力和临床实用性,并用验证集对模型预测性能进行内部验证.结果 随访69(54~71)个月后,研究对象中共新发高血压290例(27.6%).Kaplan-Meier生存曲线和多因素Cox分析显示,CHG指数、年龄、体重指数、舒张压、高血压家族史、吸烟、动脉粥样硬化、尿酸和低密度脂蛋白胆固醇(LDL-C)均为新发高血压的独立预测因子(P<0.05);采用上述预测因子构建的列线图模型,在训练集和验证集预测军事飞行人员5年新发高血压的ROC曲线下面积分别为0.872和0.849,决策曲线分析结果显示临床获益良好.结论 CHG指数是军事飞行人员高血压发病风险的独立预测因子;基于CHG指数和其他临床指标构建的列线图模型可用于早期识别高血压风险.

Objective To develop a nomogram model based on the cholesterol-high-density lipoprotein-glucose(CHG)index and clinical indicators to predict the 5-year risk of hypertension onset in military aircrew,thereby providing a predictive tool for early identification and intervention.Methods A total of 1051 normotensive male military aircrew who underwent physical examinations or clinical diagnosis and treatment at the Air Force Medical Center between January 2016 and June 2020 were retrospectively enrolled.They were randomly divided into a training set(n=736)and a validation set(n=315)and followed up until July 30,2025.Kaplan-Meier analysis and Cox proportional hazards models were used to analyze the effects of CHG index levels and other clinical indicators on hypertension incidence.A nomogram prediction model incorporating CHG index was constructed.The receiver operating characteristic(ROC)curve,calibration curve,and decision curve analysis(DCA)were applied to evaluate the model's discrimination,calibration ability,and clinical utility.Internal validation of the model's predictive performance was performed in the validation set.Results After a median follow-up of 69(interquartile range,54-71)months,a total of 290 new-onset hypertension cases(27.6%)were identified among the cohort.Kaplan-Meier survival curves and multivariable Cox regression analysis indicated that the CHG index,age,body mass index,diastolic blood pressure,family history of hypertension,smoking status,atherosclerosis,uric acid,and low-density lipoprotein cholesterol(LDL-C)were all independent predictors of new-onset hypertension(P<0.05).The nomogram model constructed with the above predictors predicted that the areas under the ROC curve of 5-year new-onset hypertension in military aircrew in the training set and the validation set were 0.872 and 0.849,respectively.The results of decision curve analysis showed favorable clinical benefit.Conclusions The CHG index is an independent predictor of hypertension incidence risk in military aircrew.The nomogram model integrating the CHG index and other clinical indicators can be used for the early identification of hypertension risk.
