高原地区极端自然环境可显著增加急性肺血栓栓塞症的发生风险.高原地区急性肺血栓栓塞症的临床表现常与高原肺水肿、肺炎等疾病重叠,且D-二聚体等常用生物标志物因高原生理适应而特异性降低,低海拔地区的临床评估工具可能不适用,加之医疗资源相对有限,导致其诊断困难及危险分层失准;治疗上虽遵循标准肺栓塞管理原则,但因高原缺氧环境下溶栓出血风险增高、患者转运延迟等特殊问题,故其诊断、鉴别诊断和治疗面临严峻挑战.因此,全军临床重点专科(高原医学)、极端环境医学教育部重点实验室、军队后勤科研重点实验室、高原高寒医学保障军队重点实验室、陆军战创伤心血管病专科联盟牵头召集全国高原医学、心血管内科、血管外科、急诊科、重症医学科、呼吸与危重症医学科等领域专家,共同制定了《高原地区急性肺血栓栓塞症诊断与治疗专家共识》.本共识包括高原地区急性肺血栓栓塞症流行病学与发病机制、诊断、治疗、预防与随访4个部分,共16条推荐意见,就危险分层、个体化治疗、预防策略及长期随访提出了具体建议,以指导高原地区特殊环境下急性肺血栓栓塞症的防治与研究工作.

Extreme natural environments in high-altitude regions can significantly increase the risk of acute pulmonary thromboembolism(PTE).Clinical manifestations of acute PTE in these regions often overlap with those of conditions such as high-altitude pulmonary edema and pneumonia.Moreover,common biomarkers including D-dimer exhibit reduced specificity due to high-altitude physiological adaptation,while clinical assessment tools validated for low-altitude populations may not be applicable here.Coupled with relatively limited medical resources,these factors lead to diagnostic difficulties and inaccurate risk stratification.Although the standard management principles for pulmonary embolism are generally followed,special problems exist,such as increased bleeding risk associated with thrombolysis in hypoxic high-altitude environments and delayed patient transfer.Therefore,the diagnosis,differential diagnosis,and treatment of this disease in high-altitude regions face severe challenges.To address these issues,the Military Clinical Key Specialty(High-Altitude Medicine),Ministry of Education Key Laboratory of Extreme Environmental Medicine,Key Laboratory of Military Logistics Scientific Research,Military Key Laboratory of High-Altitude and Cold Environment Medical Support,and the Army Trauma and Cardiovascular Disease Specialist Alliance took the lead in convening experts nationwide from fields including high-altitude medicine,cardiology,vascular surgery,emergency medicine,critical care medicine,and respiratory and critical care medicine to jointly develop the"Expert consensus on diagnosis and treatment of acute pulmonary thromboembolism in high-altitude regions".This consensus covers four sections on acute PTE in high-altitude regions:epidemiology and pathogenesis,diagnosis,treatment,as well as prevention and follow-up,with a total of 16 recommendations.It puts forward specific suggestions on risk stratification,individualized treatment,prevention strategies,and long-term follow-up,aims to guide the prevention,treatment,and research of acute PTE under special high-altitude environments.
