Chronic obstructive pulmonary disease(COPD)complicated with sarcopenia not only reduces patients'exercise tolerance and quality of life but is also closely associated with acute exacerbations and mortality of COPD.Chronic systemic inflammatory response,protein metabolism imbalance,and mitochondrial homeostasis abnormality play key roles in the progression of this disease.CHE Deya,a renowned traditional Chinese medicine physician,believes that COPD initially manifests primarily as lung deficiency,which inevitably affects the spleen and kidney over time.Although sarcopenia is located in the tendons,muscles,and flesh,its primary pathological organ is the spleen,and it is closely related to the kidney.Therefore,this article analyzes the integrated Chinese and Western medical mechanisms underlying the comorbidity of COPD and sarcopenia from the perspective of simultaneous disease of the spleen and kidney,and proposes that the basic pathogenesis of stable COPD combined with sarcopenia is spleen-kidney deficiency.The therapeutic principle is simultaneous treatment of the spleen and kidney,and herbal prescriptions for warming and tonifying the spleen and kidney to consolidate the root should be used.This paper aims to provide insights for the prevention and treatment of this comorbidity.
