目的:探讨紫朱软膏(Zizhu Ointment,ZZO)通过抑制肽基精氨酸脱氨酶4(PAD4)介导的中性粒细胞胞外陷阱(neutrophil extracellular traps,NETs)从而促进糖尿病小鼠创面愈合中的作用机制.方法:采用糖尿病创面小鼠模型,将48只C57/BL6J雄性小鼠随机分为4组,包括正常创面组、糖尿病创面组、ZZO治疗组及贝复济治疗组,每组12只.在不同时间点(0、3、7和14 d)拍照记录创面愈合情况,并分析各组创面愈合率的差异.采用H&E染色分析创面炎症细胞聚集情况;免疫荧光分析创面NETs分布;Western blot检测创面 Cit-H3/H3 比例;免疫组化法检测 PAD4、NOD 样受体基因家族 P3(NLRP3)和白细胞介素-1β(IL-1β)的蛋白表达;RT-qPCR法检测PAD4、NLRP3和IL-1β mRNA的水平.结果:糖尿病创面组创面愈合率明显低于正常创面组(P<0.01),ZZO治疗组创面愈合率明显高于糖尿病创面组(P<0.01),且在第7天愈合效果优于贝复济治疗组(P<0.05).ZZO可抑制糖尿病小鼠创面炎症细胞聚集(P<0.01)及NETs的形成(P<0.01),降低PAD4、NLRP3和IL-1β的蛋白表达(P<0.01)和mRNA的水平(P<0.01).结论:ZZO可能通过抑制PAD4介导的NETs形成以及炎症因子NLRP3和IL-1β的释放,从而促进糖尿病小鼠创面愈合.

Objective:To investigate the mechanism by which Zizhu Ointment(ZZO)promotes wound healing in diabetic mice by inhibiting neutrophil extracellular traps(NETs)mediated by peptidyl arginine deiminase 4(PAD4).Methods:A diabetic wound mouse model was used,and 48 male C57/BL6J mice were randomly divided into 4 groups:the normal wound group,the diabetic wound group,the ZZO treatment group,and the Beifuji treatment group,with 12 mice in each group.Wound healing was recorded by photography at different time points(0,3,7,and 14 days),and the differences in wound healing rates among the groups were analyzed.H&E staining was used to analyze the aggregation of inflammatory cells in the wound.Immunofluorescence was used to analyze the distribution of NETs in the wound.Western blot was used to detect the Cit-H3/H3 ratio in the wound.Im-munohistochemistry was used to detect the protein expression of PAD4,NOD-like receptor family pyrin domain containing 3(NL-RP3),and Interleukin-1β(IL-1β).RT-qPCR was used to detect the mRNA expression levels of PAD4,NLRP3,and IL-1β.Re-sults:The wound healing rate of the diabetic wound group was significantly lower than that of the normal wound group(P<0.01).The wound healing rate of the ZZO treatment group was significantly higher than that of the diabetic wound group(P<0.01),and the healing effect was better than that of the Beifuji treatment group on day 7(P<0.05).ZZO could inhibit the aggrega-tion of inflammatory cells(P<0.01)and the formation of NETs(P<0.01)in the wounds of diabetic mice,and reduce the protein expression(P<0.01)and mRNA levels(P<0.01)of PAD4,NLRP3,and IL-1β.Conclusion:ZZO may promote wound heal-ing in diabetic mice by inhibiting PAD4-mediated NETs formation and the release of inflammatory factors NLRP3 and IL-1β.
