董氏奇穴动气针法联合姿势解密技术治疗脑卒中后偏瘫肩痛的疗效观察OA
Clinical Observation on the Efficacy of Dong's Extraordinary Acupoint Motion Acupuncture Combined with Posture Decoding Technique in the Treatment of Post-Stroke Hemiplegic Shoulder Pain
[目的]观察董氏奇穴动气针法联合姿势解密技术治疗脑卒中后偏瘫肩痛(HSP)的临床疗效.[方法]选取2024年9月至2025年3月在马鞍山市中医院就诊的90例明确诊断为脑卒中后HSP的患者为研究对象.按照随机数字表法将患者随机分为观察组和对照组,每组各45例.对照组给予姿势解密技术治疗,观察组在对照组治疗的基础上,给予董氏奇穴动气针法治疗,连续治疗4周.治疗4周后,评价2组临床疗效.观察2组患者治疗前后疼痛视觉模拟评分(VAS)的变化情况,以及被动肩关节活动度(PROM)的情况.比较2组患者治疗前后Fugl-Meyer上肢运动功能评分量表(FMA-UL)评分和改良Barthel指数(MBI)评分的变化情况.[结果](1)治疗后,2组患者的VAS评分均明显改善,且观察组在改善VAS评分方面明显优于对照组,差异有统计学意义(P＜0.05).(2)治疗后,2组患者的肩关节活动度(PROM)包括肩关节前屈、外展及外旋角度均明显改善,且观察组在改善肩关节前屈、外展及外旋角度方面明显优于对照组,差异有统计学意义(P＜0.05).(3)治疗后,2组患者的FMA-UL评分、MBI评分明显改善(P＜0.05),且观察组在改善FMA-UL评分、MBI评分方面明显优于对照组,差异有统计学意义(P＜0.05).(4)观察组总有效率为95.56%(43/45),对照组为71.11%(32/45).观察组疗效优于对照组,差异有统计学意义(P＜0.05).[结论]董氏奇穴动气针法联合姿势解密技术治疗脑卒中后HSP,能明显改善患者的疼痛症状,提高患者肩关节的活动范围,增强肩关节的灵活性和稳定性,有效促进功能恢复,改善患者日常生活能力,疗效显著.
Objective To observe the clinical efficacy of Dong's extraordinary acupoint motion acupuncture combined with posture decoding technique in the treatment of post-stroke hemiplegic shoulder pain(HSP).Methods A total of 90 patients with a confirmed diagnosis of post-stroke HSP,who were treated at Maanshan Hospital of Traditional Chinese Medicine from September 2024 to March 2025,were enrolled and randomly divided into an observation group(45 cases)and a control group(45 cases)using a random number table.The control group received posture decoding technique,while the observation group received Dong's extraordinary acupoint motion acupuncture in addition to the treatment given to the control group.Both groups were treated continuously for 4 weeks.After 4 weeks of treatment,the clinical efficacy was evaluated.Changes in the Visual Analogue Scale(VAS)score for pain,passive range of motion(PROM)of the shoulder(including flexion,abduction,and external rotation),Fugl-Meyer Assessment-Upper Limb(FMA-UL)score,and Modified Barthel Index(MBI)score were observed before and after treatment.Results(1)After treatment,the VAS scores in both groups were significantly improved,and the improvement in the observation group was significantly greater than that in the control group,the difference being statistically significant(P＜0.05).(2)After treatment,the PROM of the shoulder,including the angles of flexion,abduction,and external rotation,were significantly improved in both groups,with the observation group showing significantly greater improvement than the control group,the difference being statistically significant(P＜0.05).(3)After treatment,the FMA-UL and MBI scores were significantly improved in both groups(P＜0.05),and the improvement in the observation group was significantly superior to that of the control group,the difference being statistically significant(P＜0.05).(4)The total effective rate was 95.56%(43/45)in the observation group and 71.11%(32/45)in the control group.The clinical efficacy of the observation group was significantly superior to that of the control group,the difference being statistically significant(P＜0.05).Conclusion Dong's extraordinary acupoint motion acupuncture combined with posture decoding technique can significantly alleviate pain,increase the range of motion of the shoulder joint,enhance shoulder flexibility and stability,effectively promote functional recovery,and improve the activities of daily living in patients with post-stroke HSP,demonstrating remarkable clinical efficacy.
