目的 验证远程无感智能睡眠监测系统(remotely contactless intelligent sleep monitoring system,RCISMS)指导下阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者下颌前移矫治器(mandibular advancement device,MAD)滴定治疗的疗效.方法 本研究采用随机对照设计,根据纳入与排除标准筛选后,最终共有100例中度至重度OSAHS患者入组,随机分为RCISMS客观滴定组(n=50)和主观滴定组(n=50).RCISMS客观滴定组采用RCISMS指导MAD滴定,主观滴定组采用常规方法滴定.记录治疗前后呼吸暂停低通气指数(AHI)、最低血氧饱和度(LSaO2)、Epworth嗜睡量表(ESS)等评价指标.结果 RCISMS客观滴定组治疗总有效率为80.0%(40/50),显著高于主观滴定组的56.0%(28/50),差异比较有统计学意义(χ2=6.56,P=0.010).RCISMS客观滴定组治愈率为24.0%(12/50),高于主观滴定组的12.0%(6/50).RCISMS客观滴定组AHI降低率为81.4%±9.2%,显著高于主观滴定组的46.9%±13.1%(P<0.001).滴定效率方面,RCISMS客观滴定组总滴定周期为(28.4±6.3)d,显著短于主观滴定组的(52.6±7.8)d(P<0.001),滴定效率达52.7%±10.5%.结论 RCISMS远程无感智能睡眠监测系统可显著提高OSAHS患者MAD滴定治疗效果,并大幅提升滴定效率,具有良好的临床应用价值.

OBJECTIVE To validate the efficacy of mandibular advancement device(MAD)titration guided by the remotely contactless intelligent sleep monitoring system(RCISMS)in patients with obstructive sleep apnea hypopnea syndrome(OSAHS).METHODS This study was designed as a randomized controlled trial,after applying inclusion and exclusion criteria,100 patients with moderate to severe OSAHS were enrolled and randomly assigned to the RCISMS objective titration group(n=50)or the subjective titration group(n=50).The RCISMS group underwent MAD titration guided by the RCISMS,while the control group received conventional titration.The apnea-hypopnea index(AHI),lowest oxygen saturation(LSaO₂),and Epworth Sleepiness Scale(ESS)score were recorded before and after treatment.RESULTS The overall response rate in the RCISMS objective titration group was 80.0%(40/50),significantly higher than that in the subjective titration group(56.0%,28/50),the difference was statistically significant(χ²=6.56,P=0.010).The cure rate in the RCISMS objective titration group was 24.0%(12/50),which was higher than that in the subjective titration group(12.0%,6/50).The AHI reduction rate in the RCISMS group(81.4%±9.2%)was significantly higher than that in the subjective titration group(46.9%±13.1%,P<0.001).In terms of titration efficiency,the total titration period in the RCISMS group(28.4±6.3)days was significantly shorter than that in the subjective titration group[(52.6±7.8)days,P<0.001],with a titration efficiency of 52.7%±10.5%.CONCLUSION The RCISMS can significantly improve the efficacy of MAD titration in patients with OSAHS and substantially enhance titration efficiency,demonstrating good clinical application value.
