目的 探讨季节性过敏性鼻炎(seasonal allergic rhinitis,SAR)患者花粉期与非花粉期鼻分泌物嗜酸性炎症特征,并分析相关指标与症状严重程度的关系.方法 纳入2023年12月~2025年1月于首都医科大学附属北京同仁医院就诊的162例SAR患者,按就诊时是否处于花粉期分组.采用鼻分泌物涂片进行细胞定量计数及半定量分级,并分析嗜酸性粒细胞(Eos)指标与症状评分的相关性.结果 花粉期组97例,非花粉期组65例.花粉期组鼻部症状、药物评分、外周血Eos百分比(Eos%)、Eos计数及鼻分泌物Eos%、Eos计数、脱颗粒Eos比例和计数均显著升高,而两组Eos半定量分级差异无统计学意义.鼻分泌物Eos%及脱颗粒Eos比例与多项鼻部症状评分呈正相关.结论 花粉期SAR患者全身及鼻腔局部嗜酸性炎症增强.鼻分泌物Eos定量分析,尤其是脱颗粒状态评估,可能为炎症程度和疾病活动度判断提供客观参考.

OBJECTIVE To compare eosinophilic inflammation in nasal secretions between patients with seasonal allergic rhinitis(SAR)examined during and outside the pollen season,and to assess the associations between eosinophil-related parameters and symptom severity.METHODS A total of 162 patients with SAR who attended Beijing Tongren Hospital between December 2023 and January 2025 were enrolled and divided into pollen-season and non-pollen-season groups according to whether their visit occurred during the pollen season.Nasal secretion smears were evaluated using quantitative cell counting and semiquantitative grading.Associations between eosinophil(Eos)-related parameters and symptom scores were analyzed.RESULTS The pollen-season group included 97 patients,and the non-pollen-season group included 65 patients.Nasal symptom scores,medication scores,peripheral blood Eos percentage and count,as well as nasal secretion Eos percentage,Eos count,proportion of degranulated Eos,and degranulated Eos count were all significantly higher in the pollen-season group.No significant difference was observed in semiquantitative Eos grading between the two groups.Nasal secretion Eos percentage and the proportion of degranulated Eos were positively correlated with multiple nasal symptom scores.CONCLUSION Systemic and local eosinophilic inflammation was increased in patients with SAR during the pollen season.Quantitative assessment of nasal Eos,particularly evaluation of Eos degranulation,may provide an objective reference for assessing inflammation severity and disease activity.
