人工智能辅助鼻细胞学全定量检测在慢性鼻炎中的临床效度分析OA
Clinical validity of AI-assisted fully quantitative nasal cytology in chronic rhinitis
目的 系统评价人工智能辅助鼻细胞学全定量检测(简称全定量)在慢性鼻炎中的临床效度,包括与传统半定量分级(简称半定量)结果的一致性以及与临床症状及血清学指标的关联性.方法 本研究选取2023年11月~2025年1月于首都医科大学附属北京同仁医院就诊的830例慢性鼻炎患者.采集鼻分泌物涂片,得到半定量与全定量细胞学参数[嗜酸性粒细胞(Eos)计数,Eos百分比(Eos%)和脱颗粒Eos比例(De/Eos%)],同步收集血清总免疫球蛋白E(tIgE),吸入性过敏原筛查,总鼻部症状评分(total nasal symptom score,TNSS)0~12分,总眼部症状评分(total ocular symptom score,TOSS)0~6分及视觉模拟量表(VAS)评分.采用Spearman分析评估相关性,Mann-Whitney U检验比较临床表型差异,有序Logistic回归比较症状关联性,限制性立方样条(restricted cubic splines,RCS)绘制剂量-反应关系.结果 全定量Eos计数与半定量结果高度一致(Spearman ρ=0.791,P<0.001),全定量Eos%与半定量结果中度一致(ρ=0.575).全定量Eos%与TNSS相关性最强(ρ=0.254),外周血Eos%与血清tIgE及吸入性过敏原筛查相关性最强(ρ=0.19~0.22).在哮喘分层中,仅外周血Eos%显示显著组间差异(P<0.05).有序Logistic回归显示,全定量Eos%模型(AIC=1578.2,Nagelkerke R²=0.039)优于半定量模型(AIC=1587.0,R²=0.033)和外周血Eos%模型(AIC=1603.1,R²=0.023).RCS显示Eos%与TNSS呈线性剂量-反应关系.结论 全定量Eos计数与半定量结果高度一致,全定量可替代半定量并提供连续量值和更宽的动态范围.全定量在症状关联性上略优于半定量,Eos%与TNSS症状评分间存在稳定线性关系,可连续反映鼻部症状严重程度.
OBJECTIVE To systematically evaluate the clinical validity of AI-assisted fully quantitative nasal cytology(hereinafter referred to as quantitative)in chronic rhinitis,including its consistency with traditional semi-quantitative methods,the strength of association with clinical symptoms and serological indicators,and its explanatory power for nasal symptoms.METHODS Nasal smears were collected to obtain traditional semi-quantitative grades(0-4+)and fully quantitative cytological parameters(Eos%,absolute Eos count,degranulated Eos ratio).Serum total IgE,aeroallergen-specific IgE screening,total nasal symptom score(TNSS,0-12),total ocular symptom score(TOSS,0-6),and individual visual analog scale(VAS)scores were simultaneously recorded.Spearman correlation was used to assess correlations,Mann-Whitney U test to compare clinical phenotypic differences,ordinal logistic regression to compare symptom explanatory power,and restricted cubic splines(RCS)to delineate dose-response relationships.RESULTS The fully quantitative Eos count was highly correlated with the semi-quantitative method(Spearman ρ=0.791,P<0.001),while Eos%showed a moderate correlation(ρ=0.575).Fully quantitative Eos%had the strongest correlation with TNSS(ρ=0.254),whereas blood Eos%correlated most strongly with total IgE and aeroallergen-specific IgE screening(ρ=0.19-0.22).In asthma stratification,only blood Eos%showed a significant inter-group difference(P<0.05).Ordinal logistic regression showed that the fully quantitative Eos%model(AIC=1578.2,Nagelkerke R²=0.039)outperformed the semi-quantitative model(AIC=1587.0,R²=0.033)and the blood Eos%model(AIC=1603.1,R²=0.023).RCS revealed a linear dose-response relationship between Eos%and TNSS.CONCLUSION The fully quantitative Eos count is highly consistent with the semi-quantitative method.AI-assisted fully quantitative testing can replace the semi-quantitative approach by providing continuous values and a wider dynamic range.Fully quantitative nasal cytology outperforms traditional semi-quantitative methods in symptom explanatory power,and a stable linear relationship exists between Eos%and TNSS,which can continuously reflect the severity of nasal symptoms.
