基于"菌群-代谢-免疫"轴从"浊毒"论治腹泻型肠易激综合征OA
Treatment of diarrhea-predominant irritable bowel syndrome from the perspective of turbid toxins based on the microbiota-metabolism-immunity axis
腹泻型肠易激综合征(IBS-D)发病机制尚未阐明,发生发展与肠道菌群紊乱、代谢失调、免疫失衡密切相关,是菌群、代谢、免疫交互联动的结果.中医认为腹泻型肠易激综合征病机关键是肝脾失调.李佃贵提出"浊毒理论",认为肝郁脾虚、浊毒内蕴为腹泻型肠易激综合征的主要病机.基于"菌群-代谢-免疫"轴,从浊毒理论出发,探讨二者之间的关联性.肠道菌群失调与脾虚具有生理相似性,肠道菌群失调导致代谢紊乱、激活免疫炎症反应等与浊毒致病特点符合,通过疏肝健脾、化浊解毒调控"菌群-代谢-免疫"轴治疗腹泻型肠易激综合征具有一定价值.
The pathogenesis of diarrhea-predominant irritable bowel syndrome(IBS-D)has not yet been fully elucidated.Its development and progression are closely related to intestinal flora imbalance,metabolic dysregulation,and immune dysfunction,resulting from the interactive interplay of microbiota,metabolism,and immunity.Traditional Chinese Medicine(TCM)posits that the key pathogenesis of IBS-D lies in liver-spleen disharmony.Li Diangui proposes the"Turbid Toxin Theory,"suggesting that liver stagnation-spleen deficiency and internal accumulation of turbid toxins constitute the primary pathogenesis of IBS-D.Based on the"gut microbiota-metabolism-immunity"axis and starting from the turbid-toxin theory,this study explores the correlation between the two.Gut microbiota dysbiosis shares physiological similarities with spleen deficiency.The metabolic disorders and activated immune-inflammatory responses caused by gut microbiota dysbiosis align with the pathogenic characteristics of turbid toxin.Regulating the"gut microbiota-metabolism-immunity"axis by soothing the liver,strengthening the spleen,resolving turbidity,and detoxifying has certain value in treating IBS-D.
