基于"脑-肠轴"探讨"运土疏木"理论治疗糖尿病共病抑郁症机制OA
Discussion on the mechanism of treating diabetes-depression comorbidity with the theory of transporting earth and dredging wood based on the brain-gut axis
基于"土壅木郁"病机,提出应用"运土疏木"法治疗糖尿病共病抑郁症,探讨其与"脑-肠轴"调节的内在关联.糖尿病共病抑郁症"土壅木郁"病机与现代医学"菌群失衡导致肠屏障受损、引发慢性炎症,进而经"脑-肠轴"诱发抑郁症并加重代谢紊乱"机制相通,治疗关键为运土(健脾化湿)以调节肠道菌群、降低炎症反应,结合疏木(疏肝行气)以调节情志与神经内分泌,改善糖尿病共病抑郁症临床症状.通过中西医理论互证,揭示运土疏木法调治糖尿病共病抑郁症的科学内涵,为建立"脑-肠轴"干预提供系统性理论支撑.
Based on the pathogenesis of"earth stagnation leading to wood depression",this study proposes the application of the"transporting earth and dredging wood"method for treating diabetes-depression comorbidity,exploring its intrinsic connection with the regulation of the"gut-brain axis".The pathogenesis of"earth stagnation leading to wood depression"in diabetes-depression comorbidity aligns with the modern medical mechanism of"gut dysbiosis leading to intestinal barrier damage,triggering chronic inflammation,which then induces depression and exacerbates metabolic disorders via the gut-brain axis."The key treatment involves"transporting earth"(strengthening the spleen and resolving dampness)to regulate gut microbiota and reduce inflammatory responses,combined with"dredging wood"(soothing the liver and promoting qi flow)to modulate mood and neuroendocrine function,thereby improving clinical symptoms of diabetes-depression comorbidity.Through mutual verification of Chinese and Western medical theories,this study reveals the scientific connotation of the"transporting earth and dredging wood"method in treating diabetes-depression comorbidity,providing systematic theoretical support for establishing"brain-gut axis"interventions.
