慢传输型便秘是肛肠科常见的难治性疾病,缠绵难愈,严重影响患者生活质量.现代医学认为慢传输型便秘主要与结直肠神经肌肉功能失调、慢性炎症、肠道微生物组改变等有关.中医学"膜络一体"理论认为膜系脉络网状系统在功能上与"脾土"相契合,其功能发挥的物质基础赖于"脾阴".立足脾阴虚,从整体理论出发,以膜系络病理论为指导,探讨"中虚便变"的病因病机,认为"中虚"运化转输失职,升清降浊枢机不利,初则膜络气血怫郁,气、液功能失调,久则络变膜损,终致肠腑传导失司,发生"便变".治疗慢传输型便秘当以甘淡理脾阴为第一要法,兼顾命门,辅以疏利三焦膜系.

Slow transit constipation is a common refractory disease in proctology,often persistent and difficult to cure,seriously affecting the overall quality of patients'life.Modern medicine believes that it is mainly related to colorectal neuromuscular dysfunction,chronic inflammation,changes in the intestinal microbiome,etc.The view of"integration of membrane and collaterals"in traditional Chinese medicine holds that the reticular system of the membrane system and collateral is functionally consistent with the"spleen-earth",with its material basis for its functional implementation relying on the"spleen yin".Based on the theory of spleen yin deficiency,starting from the holistic theory,and guided by the theory of membrane and collateral diseases,this paper explores the etiology and pathogenesis of"Zhong Xu Bian Bian".This study posits that"Zhong Xu"leads to impaired transportation and transformation,and the obstruction of the pivot for ascending the clear and descending the turbid.Initially,the qi and blood in the membrane and collateral are stagnated,and the functions of qi and fluid are disordered;Prolonged conditions result in the collaterals change and the membrane damage,ultimately leading to the dysfunction of the intestine conduction and the occurrence of"Bian Bian".For the treatment strategy of slow transit constipation,regulating the spleen yin with sweet and mild herbs should be regarded as the primary method,while also considering ming men,supplemented by dredging sanjiao membrane system.
