Endometriosis is a refractory gynecological disorder characterized most prominently by dysmenorrhea.Professor Wang Qingguo proposes that dysfunction of the shaoyang pivot is a key pathological link in the development of dysmenorrhea associated with endometriosis.When the shaoyang pivot functions smoothly,qi transformation among the three yang meridians proceeds in an orderly manner;when the qi transformation by the pivot is impaired,disharmony between the exterior and interior occurs,accompanied by disordered distribution of qi,blood,and body fluids.The relationship between the concept of"shaoyang as the pivot"and endometriosis-related dysmenorrhea is mainly reflected in the following aspects:impaired pivot function leading to an insufficient source of qi and blood;deficiency of yang qi result ing in gradual accumulation of cold pathogens;and qi stagnation giving rise to blood stasis,ultimately forming abdominal masses and accumulations.Based on this understanding,the"three methods for regulating the pivot"are applied in the treatment of endometriosis-related dysmenorrhea,namely:promoting the free flow of the pivot function to nourish blood and supplement qi;warming the pivot and dispersing cold to support yang and eliminate accumulation;and activating blood and unblocking the pivot to resolve masses and stagnation.This article further elucidates Professor Wang's clinical experience in treating dysmenorrhea due to endometriosis with the"three methods for regulating the pivot"through the analysis of representative clinical cases.
