目的 明确四逆散对Graves 病模型小鼠的干预作用,从免疫中枢(脾脏)、体液循环(血清)及靶器官(甲状腺)3 个层面探讨四逆散对Th1/Th2/Th17 细胞免疫失衡的调控机制.方法 将24 只雌性BALB/c 小鼠按随机数字表法分为正常组(n=6)及造模组(n=18),采用肌内注射 Ad-TSHR289 腺病毒的方法复制Graves 病模型.将成模小鼠按随机数字表法分为模型组、甲巯咪唑组(4.55×10-3 g/kg)及四逆散组(5.46g/kg),并灌胃相应药物,每日1 次,连续6 周.使用 HE 染色法观察甲状腺滤泡上皮细胞病理改变;放射免疫法检测血清促甲状腺激素受体抗体(TRAb)、甲状腺素(T4)水平;流式细胞术检测脾脏组织Th1、Th2、Th17 细胞占比;定量反转录 PCR 法和蛋白质印迹法检测脾脏组织 Th1 细胞分泌的γ 干扰素(IFN-γ)、Th2 细胞分泌的白细胞介素(IL)-4 及Th17 细胞分泌的IL-17A 的mRNA与蛋白表达;Luminex 蛋白生物芯片检测血清 IFN-γ、IL-4、IL-10、IL-17A,以及调节正常 T 细胞表达和分泌的活化因子(RANTES)、巨噬细胞炎症蛋白-1β(MIP-1β)、单核细胞趋化蛋白-1(MCP-1)的水平;免疫组织化学法检测甲状腺组织IFN-γ、IL-4、IL-17A 定位及表达.结果 四逆散可减小模型小鼠甲状腺体积,降低血清TRAb 和T4 水平(P<0.01),改善甲状腺滤泡结构病理损伤,减轻脾脏肿大.机制研究结果发现,与正常组比较,模型组脾脏组织 Th1 细胞占比、Th17 细胞占比及 Th1/Th2 比值有上升趋势,Th2 细胞占比有下降趋势;脾脏组织 IFN-γ、IL-17A mRNA 表达及 IFN-γ 蛋白表达上调(P<0.01),IL-17A 蛋白表达呈上调趋势,IL-4 mRNA 表达下调(P<0.01);血清IFN-γ、IL-10、RANTES 水平升高(P<0.01),IL-17A、MIP-1β、MCP-1 水平呈升高趋势,IL-4 水平呈降低趋势;甲状腺组织 IFN-γ、IL-4、IL-17A 蛋白阳性表达水平均有所升高,提示 Graves 病模型小鼠存在 Th1/Th2/Th17 细胞免疫失衡.甲巯咪唑及四逆散可调节Graves 病模型小鼠脾脏、甲状腺及血清 IFN-γ、IL-4、IL-17A 等表达,亦可调节RANTES、MIP-1β、MCP-1等趋化因子表达.结论 四逆散可能通过调控 Th1/Th2/Th17 细胞免疫失衡及RANTES、MIP-1β、MCP-1趋化因子表达而干预Graves 病所致的甲状腺肿大.

Objective To investigate the effects of Sini Powder on Graves' disease(GD)model mice and to explore its mechanism in regulating Th1/Th2/Th17 cells immune imbalance at three levels:immune center(spleen),humoral circulation(serum),and target organ(thyroid).Methods Twenty-four female BALB/c mice were randomly divided into normal group(n=6)and modeling group(n=18)using a randomized block design.GD model was established through intramuscular injection of Ad-TSHR289 adenovirus.After successful modeling,GD mice were randomly assigned to the model,methimazole(4.55×10-3 g/kg),and Sini Powder(5.46 g/kg)groups using random number table,with corresponding interventions administered daily for 6 weeks.HE staining was used to assess pathological changes in thyroid follicular epithelial cells;serum levels of thyrotropin receptor antibody(TRAb)and thyroxine(T4)were measured using radioimmunoassay.The proportions of Th1,Th2,and Th17 cells in spleen tissues were analyzed by flow cytometry;mRNA and protein expressions of Th1-derived interferon-γ(IFN-γ),Th2-derived interleukin-4(IL-4),and Th17-derived interleukin-17A(IL-17A)in spleen were detected by quantitative reverse transcription PCR and Western blotting.Serum levels of IFN-γ,IL-4,IL-10,IL-17A,as well as regulated on activation,normal T-cell expressed and secreted(RANTES),macrophage inflammatory protein-1β(MIP-1β),and monocyte chemoattractant protein-1(MCP-1)were measured using Luminex protein biochip assay.Immunohistochemistry was performed to localize and assess the expression of IFN-γ,IL-4,and IL-17A in thyroid tissues.Results Sini Powder reduced thyroid volume of model mice,as well as decreased serum TRAb and T4 levels(P<0.01),ameliorated pathological damage to thyroid follicular structures,and alleviated splenomegaly.Mechanistic studies revealed that compared with the normal group,the model group exhibited increased trends of the proportions of Th1 and Th17 cells,along with an elevated Th1/Th2 ratio,whereas Th2 cell proportion decreased.Expressions of IFN-γ and IL-17A mRNA,as well as IFN-γ protein expression in spleen increased(P<0.01),with an upward trend in IL-17A protein expression,while IL-4 mRNA expression was downregulated(P<0.01).The serum levels of IFN-γ,IL-10,and RANTES elevated(P<0.01),with upward trends in IL-17A,MIP-1β,and MCP-1,and a downward trend in IL-4 level.The expressions of IFN-γ,IL-4,and IL-17A in thyroid tissues increased.These findings suggest the presence of Th1/Th2/Th17 cells immune imbalance in GD model mice.The methimazole and Sini Powder modulated expressions of IFN-γ,IL-4,and IL-17A in spleen,thyroid,and serum,as well as regulated the expression of chemokines including RANTES,MIP-1β,and MCP-1 of the GD model mice.Conclusion Sini Powder may exert intervention effect on goiter induced by GD via regulating Th1/Th2/Th17 cells immune imbalance and modulating chemokines such as RANTES,MIP-1β,and MCP-1.
