Endophthalmitis after cataract surgery:analysis of clinical characteristics and risk factorsOA
Purpose:To investigate the ten-year incidence of postoperative endophthalmitis(POE)after cataract surgery and to identify the preoperative and intraoperative risk factors.Methods:This retrospective case–control study included all patients diagnosed with POE after phacoemulsification at the Zhongshan Ophthalmic Centre,Sun Yat-sen University,China between 2014 and 2024.Each case was randomly matched with 3 patients without endophthalmitis who underwent surgery by the same surgeon on the same date.Clinical variables were extracted from the electronic surgical database and compared between case and control groups.Annual trends in incidence were evaluated across the study period.Univariate and multivariate logistic regression analysis were performed to assess associated risk factors.Results:Among 211,740 cataract surgeries,25 patients(0.012%)were diagnosed with POE.Statistically significant differences between the case and control groups were observed for male proportion,history of prior surgery in the operated eye,incidence of intraoperative complications,and concomitant procedures(all P<0.05).Multivariate logistic regression indicated that male(OR=2.598),Charlson comorbidity score(OR:1.625),and intraoperative complications(OR=18.317)were significant risk factors for postoperative endophthalmitis(all P<0.05).The pathogen culture positive rate was 24%(6/25).Conclusions:The incidence of POE following cataract surgery remained low over the decade.Postoperative attention should be directed toward male patients and those with higher Charlson comorbidity scores and intraoperative complications.These results highlight the need for continued monitoring and multicenter surveillance in China.
