近视发生发展中视网膜分子信号通路的调控机制及干预研究进展OA
Research progress on the regulatory mechanism and intervention of molecular signaling pathways in the retina during the oc-currence and development of myopia
近视已成为全球范围内日益严峻的公共卫生问题,其发病机制复杂,涉及多种信号通路的交互作用.视网膜作为视觉信号感知和转导的起点,在启动近视级联反应中发挥核心作用,通过感知视觉环境变化,激活下游多条分子信号通路,调控细胞增殖、凋亡、基质代谢及炎症反应,并通过"视网膜-脉络膜-巩膜"信号轴介导巩膜重塑,进而诱导眼轴异常延长,促进近视发生与进展.本文系统综述了多巴胺、Wnt/β-catenin、ON/OFF、MAPK/ERK、TGF-β/Smad及炎症相关等关键分子信号通路在近视中的调控机制,阐述了表观遗传调控在视网膜信号通路中的作用,并总结了基于视网膜信号通路的药物、基因、光环境及细胞治疗等干预策略,为深入解析近视视网膜发病机制、明确近视发病的分子靶点、开发新型靶向精准防控手段提供理论依据与研究方向.
Myopia has emerged as a significant global public health challenge,characterized by a complex pathogen-esis involving intricate interactions among multiple signaling pathways.As the primary site for visual signal perception and transduction,the retina plays a central role in initiating the myopic cascade.It detects changes in the visual environment,activates downstream molecular signaling pathways,and regulates cellular processes such as proliferation,apoptosis,extra-cellular matrix metabolism,and inflammatory responses.Through the retina-choroid-sclera signaling axis,the retina medi-ates scleral remodeling,thereby inducing abnormal elongation of the eye axis and promoting the occurrence and progression of myopia.This review systematically outlines the regulatory mechanisms of key signaling pathways—including dopamine,Wnt/β-catenin,ON/OFF,MAPK/ERK,TGF-β/Smad,and inflammation-related pathways—in myopia development.It fur-ther elucidates the role of epigenetic regulation in retinal signaling and summarizes intervention strategies based on retinal pathways,such as pharmacological,genetic,optical environment modulation,and cell-based therapies.This synthesis pro-vides a theoretical foundation and research directions for further understanding retinal mechanisms in myopia pathogene-sis,identifying molecular targets,and developing targeted precision interventions.
