Comparative analysis of choroidal circulation changes in varied tilted postures

目的 探讨不同倾斜体位下脉络膜血液循环的变化差异,分析体位改变对脉络膜厚度(SFCT)及脉络膜血管指数(CVI)的影响.方法 招募21名健康受试者(39眼),采用扫频源光学相干断层扫描血管成像(SS-OCTA)技术,测量受试者在坐姿基线、15°头高脚低位、平卧位、15°头低脚高位及复位后不同时间点的SFCT和CVI.通过重复测量方差分析(ANOVA)比较不同体位及时间点的参数变化.结果 体位改变显著影响SFCT和CVI.头低脚高位15°时,SFCT和CVI在1分钟内分别增加15.5%和16.8%,10分钟达峰值(SFCT增幅20.0%,CVI增幅19.4%).所有体位复位后30分钟,参数均恢复至基线水平(P>0.05).ANOVA显示体位、时间及交互效应均显著(P<0.001).结论 体位变化可逆性改变脉络膜血流动力学,头低位显著增加SFCT和CVI,可能与重力驱动的静脉压变化及血管自身调节有关.

Objective To investigate the differences in choroidal blood circulation under various tilted positions and analyze the effect of postural changes on subfoveal choroidal thickness(SFCT)and choroidal vascularity index(CVI).Methods Twenty-one healthy subjects(39 eyes)were recruited.Using swept-source optical coherence tomography angi-ography(SS-OCTA),SFCT and CVI were measured at the following time points:baseline sitting position,15° head-up tilt,supine position,15° head-down tilt,and after returning to the baseline position.Repeated-measures analysis of vari-ance(ANOVA)was used to compare parameter changes across different positions and time points.Results Postural changes significantly affected SFCT and CVI.At 15° head-down tilt,SFCT and CVI increased by 15.5%and 16.8%,re-spectively,within 1 minute,peaking at 10 minutes(20.0%increase in SFCT and 19.4%increase in CVI).All parame-ters returned to the baseline level 30 minutes after returning to the baseline position(P>0.05).ANOVA showed signifi-cant effects for position,time,and their interaction(P<0.001).Conclusion Postural changes reversibly alter choroidal hemodynamics.The head-down position significantly increases SFCT and CVI,which may be related to gravity-induced venous pressure changes and vascular autoregulation.
