基于"阳化气,阴成形"理论探讨针刺治疗变应性鼻炎的思路OA

Allergic rhinitis(AR)is a chronic non-infectious inflammation of the nasal mucosa mediated by allergen-specific immunoglobulin E(sIgE).It is mainly characterized by nasal itching,nasal congestion,clear nasal discharge and paroxysmal sneezing.In traditional Chinese medicine(TCM),it is categorized into the scope of"Bi qiu"(a TCM-specific term corresponding to AR,characterized by nasal congestion and profuse clear discharge).Based on the TCM theory of"yang transforming qi and yin constituting form"and combined with the Th1/Th2 imbalance mechanism in modern immu-nology,this paper points out that the core pathogenesis of AR lies in the insufficiency of"yang transforming qi"in the three zang-organs(lung,spleen and kidney)as the root cause,and the excessive"yin constituting form"of water-damp-ness,phlegm and fluid retention as the branch cause.These two factors form a vicious cycle of"deficiency of the root gen-erating pathogenic factors,and hyperactivity of pathogenic factors damaging healthy qi".Under the guidance of this theo-ry,an acupuncture treatment strategy of"taking assisting yang to transform qi as the main approach and eliminating phlegm and removing blood stasis as the auxiliary method"is proposed.Specifically,the yang qi of zang-organs is regulated and replenished through three TCM therapeutic methods:"reinforcing earth to generate metal"(pei tu sheng jin),"mutual generation of metal and water"(jin shui xiang sheng),and"strengthening yang and dredging the governor vessel"(fu yang tong du).Meanwhile,the method of eliminating phlegm and removing blood stasis is adopted to intervene in the concurrent syndrome of phlegm and blood stasis.This study provides a theoretical basis and clinical scheme for the acupuncture treat-ment of AR.
