The theory of sovereign fire(jun huo)and ministerial fire(xiang huo)is animportant compo-nent of the theoretical framework of traditional Chinese medicine.Based on the principle that"the sovereign fire governs clarity and the ministerial fire fulfills its role",Professor CHANG Ke approaches the understand-ing and treatment of pediatric tic disorder from the perspective of exuberant sovereign fire and displaced minis-terial fire.The etiology involves wind,fire,phlegm,stasis,and deficiency,while the pathogenesis is charac-terized by exuberant sovereign fire and displacement of ministerial fire,leading to a state in which the mind de-sires tranquility but fire fails to subside.For cases with exuberant sovereign fire blazing alone upward,treat-ment should focus on clearing the heart,draining fire,calming the spirit,and suppressing tics,using a modi-fied Daochi San(导赤散)combined with Yehua Tang(野化汤).For cases with accumulated heat in the spleen and stomach leading to loss of sovereign fire clarity,treatment should focus on clearing heat from the spleen,draining fire,and unblocking the middle energizer,using a modified Qingre Xiepi San(清热泻脾散).For cases with kidney yin deficiency leading to displaced ministerial fire,treatment should focus on nourishing yin,reducing fire,calming the liver and kidney,using a modified Zhibai Dihuang Wan(知柏地黄丸).The ulti-mate goal is to clarify the sovereign fire and stabilize the ministerial fire,allowing both to perform their respec-tive functions without excessive fire,thereby stopping the tics and restoring normalcy.One representative case is attached for validation.
