基于"玄府郁闭"理论探讨风药开玄分期论治痤疮OA

In response to the high recurrence rate and significant side effects associated with Western medical treatments for acne,as well as the tendency of traditional Chinese medicine treatments to"clear heat at the expense of healthy qi and dispel stasis while consuming qi",this paper,based on Liu Wansu's theory of"xuanfu,qi,and fluid",proposes that"obstruction of the cutaneous xuanfu"is the core pathogenesis of acne.Its progression follows the pathway of"initial obstruction generating stagnant heat → congestion leading to phlegm-stasis → exhaustion resulting in deficiency-stasis".Accordingly,a stage-based treatment system of"unblocking the xuanfu with wind-dispersing medicinals"is constructed:in the early stage,medicinals such as Mentha haplocalyx(Bohe)and Schizonepeta tenuifolia(Jingjie)are used to open the xuanfu and clear heat;in the middle stage,Saposhnikovia divaricata(Fangfeng)and Ligusticum chuanxiong(Chuanxiong)are employed to open the xuanfu,resolve stasis,and remove dampness;in the later stage,Bupleurum chinense(Chaihu)and Pueraria lobata(Gegen)are selected to open the xuanfu and nourish yin.Wind-dispersing medicinals function through"directly unblocking obstructions,regulating qi movement,guiding medicinals to enhance efficacy,and preventing tonic-induced stagnation".Typical clinical cases demonstrate that this system significantly reduces skin lesions and inflammation,with no adverse reactions and a low recurrence rate.It provides a new traditional Chinese medicine approach for acne treatment that balances efficacy and safety,offering important reference value for clinical practice.
