从"肝郁血瘀"论治老年失眠合并焦虑抑郁理论构建与临床实践OA

With increasing age,sleep problems among the elderly have become increasingly prominent.Statistics show that the prevalence of sleep disturbances in individuals aged 65 and above is as high as 73.7%.Difficulty falling asleep,frequent nocturia-induced awakenings,and light sleep are the main manifestations affecting sleep in the elderly.Factors such as retirement,loneliness,and chronic diseases often lead to emotional distress,resulting in liver-qi stagnation,which may further induce blood stasis and ultimately disturb the mind,forming the pathological chain of"liver-qi stagnation and blood stasis pattern".In clinical practice,most elderly individuals with insomnia also exhibit symptoms of anxiety and depression,often manifested as nervousness,worry,tendency to overthink,daytime fatigue,reduced interest,decline in memory and attention,and in severe cases,impaired cognitive function.This mechanism is closely related to traditional Chinese medicine theories such as"the liver governs free flow"and"qi commands blood".By integrating classical Chinese medicine theories with modern research,this article explores the pathological mechanisms of senile insomnia complicated with anxiety and depression from the perspectives of neuroendocrine mechanisms and hemorheological abnormalities,and verifies the clinical efficacy of Xuefu Zhuyu Decoction.
