目的 探讨新诊断2型糖尿病(type 2 diabetes mellitus,T2DM)患者早期动脉粥样硬化的代谢特征.方法 选取2019年8月至2020年7月于苏州大学附属常州老年病医院诊治的T2DM患者198例,采用高频超声测量双侧颈总动脉、股动脉及髂总动脉的内膜中层厚度(intima-media thickness,IMT),依据是否出现斑块或IMT增厚将患者分为亚临床动脉粥样硬化(subclinical atherosclerosis,subAS)组(106例)和单纯T2DM组(92例),比较两组患者的临床特征,采用二元Logistic回归分析新诊断T2DM患者合并subAS的影响因素.结果 198例患者中subAS检出率为53.54%(106/198).subAS组患者的年龄显著大于单纯T2DM组,有吸烟史、高血压病史的比例及收缩压、体质量指数(body mass index,BMI)、糖化血红蛋白(glycosylated hemoglobin A1c,HbA1c)、餐后2h血糖及甘油三酯水平均显著高于单纯T2DM组(P<0.05).二元Logistic回归分析显示,年龄、吸烟史、高血压病史、BMI及HbA1c均是新诊断T2DM患者发生subAS的独立危险因素(P<0.05).结论 新诊断T2DM患者的subAS患病率较高,关口前移、多管齐下的策略有望在症状出现前阻断AS进展,改善T2DM患者的远期预后.

Objective To explore the metabolic characteristics of early atherosclerosis in patients with newly diagnosed type 2 diabetes mellitus(T2DM).Methods A total of 198 patients with T2DM who were diagnosed and treated at Changzhou Geriatric Hospital Affiliated to Soochow University from August 2019 to July 2020 were selected.High-frequency ultrasound was used to measure the intima-media thickness(IMT)of the bilateral common carotid arteries,femoral arteries,and common iliac arteries.Based on the presence of plaques or IMT thickening,patients were divided into subclinical atherosclerosis(subAS)group(106 cases)and simple T2DM group(92 cases).Clinical characteristics were compared between two groups,and binary Logistic regression was performed to identify factors influencing subAS in newly diagnosed T2DM patients.Results The detection rate of subAS among the 198 patients was 53.54%(106/198).The age of patients in subAS group was significantly older than that in simple T2DM group,proportions of patients with a history of smoking and hypertension,as well as the levels of systolic blood pressure,body mass index(BMI),glycosylated hemoglobin A1c(HbA1c),2-hour postprandial plasma glucose and triglycerides were all significantly higher than those in simple T2DM group(P<0.05).Binary Logistic regression analysis showed that age,smoking history,history of hypertension,BMI,and HbA1c were independent risk factors for subAS in newly diagnosed T2DM patients(P<0.05).Conclusion The prevalence of subAS in newly diagnosed T2DM patients is relatively high.A strategy of moving the checkpoint forward and taking multiple approaches is expected to block the progression of AS before symptoms appear and improve the long-term prognosis of T2DM patients.
